参芪扶正注射液辅助治疗慢性阻塞性肺疾病合并右心衰竭疗效分析

Effect of Shenqi Fuzheng Injection on COPD Complicated with Right Heart Failure

目的观察参芪扶正注射液辅助治疗慢性阻塞性肺疾病(COPD)合并右心衰疗效。方法收集COPD合并右心衰竭患者104例,随机分为两组,对照组予常规治疗,观察组加用参芪扶正注射液。治疗10 d后所有患者均行超声心动图检查,评估右心室TEI指数,测量参数包括Em/Am、Am、Em、Sm、Tei。检测治疗前后血氧饱和度、收缩压、心率、呼吸频率、BNP、PaCO2水平;记录两组治疗前后肺功能指标中第1秒用力呼气量(FEV1)、第1秒呼气量占用肺活量百分比(FEV1/FVC)、FEV1占预计值百分比。记录呼吸困难缓解时间,用SGRQ量表评估生存质量。结果治疗后,观察组Tei较对照组低,其余指标均较对照组高(P<0.05);治疗后,观察组血氧饱和度升高且高于对照组,收缩压、心率、呼吸频率、BNP、PaCO2降低且低于对照组(P<0.05);治疗后观察组FEV1、FEV1/FVC、FEV1占预计值百分比均高于对照组(P<0.05);观察组呼吸困难缓解时间较对照组短(P<0.05);观察组治疗后SGRQ分值较对照组低(P<0.05)。结论参芪扶正注射液辅助治疗COPD合并右心衰竭能有效改善患者肺功能和心功能,更快缓解呼吸困难,提高患者生活质量。

Objective:To analyze the efficacy of Shenqi Fuzheng Injection on COPD complicated with right heart failure.Methods:104 patients was randomly divided into 2 groups.The control group received conventional treatment.The observation group was supplemented with Shenqi Fuzheng Injection.After 10 days of treatment,all patients were examined by echocardiography to evaluate TEI index of right ventricle,including Em/Am,Am,Em,Sm and Tei.Blood oxygen saturation,systolic blood pressure,heart rate,respiratory rate,BNP and PaCO2 levels were measured;FEV1,FEV1/FVC and FEV1 percentage of predicted value were recorded before and after treatment.Remission time of dyspnea was recorded and quality of life was assessed with SGRQ scale.Results:After treatment,Tei in the observation group was lower,and other indicators were higher than those in the control group(P<0.05);the oxygen saturation in the observation group increased and was higher than that in the control group;the systolic blood pressure,heart rate,respiratory rate,BNP and PaCO2 decreased and were lower than those in the control group(all P<0.05);FEV1,FEV1/FVC and FEV1 percentage of predicted value in the observation group were higher than those in the control group(P<0.05);the relief time of dyspnea in the observation group was shorter than that in the control group(P<0.05);the SGRQ score of the observation group was lower than that of the control group(P<0.05).Conclusion:Shenqi Fuzheng Injection can effectively improve the pulmonary function and cardiac function of patients with chronic obstructive pulmonary disease combined with right heart failure,alleviate dyspnea faster and improve the quality of life of patients.
