滋阴解毒化瘀方辅助西医对症干预治疗胸部肿瘤放疗后继发急性放射性肺炎临床研究

Therapeutic Effect of Intervention Treatment of Ziyin Jiedu Huayu Decoction Assisting Western Medicine on Acute Radiation Pneumonitis Secondary to Radiotherapy of Chest Tumor

目的探讨滋阴解毒化瘀方辅助西医对症干预治疗胸部肿瘤放疗后继发急性放射性肺炎临床疗效。方法患者84例以随机数字表法分为对照组与观察组各42例,分别采用西医规范对症方案和在此基础上辅以滋阴解毒化瘀方治疗,比较两组近期疗效,治疗前后中医证候评分、Karnofsky(KPS)评分、SF-36生活质量量表(SF-36)评分、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及转化生长因子-β(TGF-β)水平。结果观察组近期治疗总有效率显著高于对照组(P<0.05);观察组治疗后中医证候评分、KPS评分、躯体角色评分、心理健康评分及疼痛评分均显著优于对照组及本组治疗前(P<0.05);同时观察组治疗后IL-6、TNF-α及TGF-β水平均显著低于对照组及本组治疗前(P<0.05)。结论滋阴解毒化瘀方辅助西医对症干预治疗胸部肿瘤放疗后继发急性放射性肺炎可有效消除肺部炎性病灶,改善相关临床症状,提高生存质量,且在下调IL-6、TNF-α及TGF-β表达方面更具优势。

Objective:To discuss the clinical effect of intervention treatment of Ziyin Jiedu Huayu Decoction assisting Western Medicine on acute radiation pneumonitis secondary to radiotherapy of chest tumor.Methods:84 patients were randomly divided into the control group and the observation group,42 cases in each group.The patients were treated with Western Medicine and Ziyin Jiedu Huayu Decoction,respectively.The short-term effects,TCM syndrome score,KPS score,SF-36 score and levels IL-6,TNF-αand TGF-βbefore and after treatment were compared between groups.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05);TCM syndrome score,KPS score,physical role score,mental health score and pain score of the observation group were significantly higher than those of the control group and before treatment(P<0.05).Meanwhile,levels including IL-6,TNF-αand TGF-βin the observation group were significantly lower than those in the control group and before treatment(P<0.05).Conclusion:Ziyin Jiedu Huayu Decoction can effectively eliminate the inflammatory focus of lung,and improve the related clinical symptoms and the quality of life,with more advantages in down regulating the expression of IL-6,TNF-αand TGF-β.
