乙肝肝硬化患者凝血功能四项的检测价值分析

Analysis of the Detection Value of Four Coagulation Functions in Patients with Hepatitis B Cirrhosis

目的分析凝血功能四项指标在慢性乙型肝炎肝硬化患者中的检测价值。方法纳入2018年5月至2019年5月在我院肝胆科接受治疗的93例慢性乙型肝炎肝硬化患者作为观察对象(观察组),同时选取同期在我院接受体检的38例健康者作为对照对象(对照组),比较两组凝血功能四项指标差异,并分析不同肝功能分级患者凝血四项指标差异。结果观察组凝血活酶时间(APTT)、凝血酶原时间(PT)和凝血酶时间(TT)均显著高于对照组,纤维蛋白原(Fib)含量显著低于对照组,两组具有极显著差异(P<0.001);APTT、PT和TT随着肝功能分级的加重而显著增高,Fib随着肝功能分级的加重而显著降低,A、B、C三级各指标之间存在极显著差异(P<0.01)。结论凝血功能四项指标对慢性乙型肝炎肝硬化诊断有重要检出意义,可对肝功能损伤程度和治疗预后进行判断,临床检测意义显著。

Objective To analyze the detection value of four indicators of coagulation function in patients with chronic hepatitis B cirrhosis.Methods 93 patients with chronic hepatitis B cirrhosis treated in our hospital from May 2018 to May 2019 were selected as the observation subjects(observation group),and 38 healthy subjects underwent physical examination in our hospital at the same time were selected as control subjects(control group).The differences of the four indicators of coagulation function between the two groups were compared,and the differences of four indexes of coagulation in patients with different liver function grading were analyzed.Results The thromboplastin time(APTT),prothrombin time(PT)and thrombin time(TT)were significantly higher in the observation group than that of in the control group.The fibrinogen(Fib)content was significantly lower than that of in the control group(P<0.001).APTT,PT and TT increased significantly with the increase of liver function grading.Fib decreased significantly with the increase of liver function grading.There were significant differences among the three indexes of A,B and C(P<0.01).Conclusions The four indexes of coagulation function have important detection significance for the diagnosis of chronic hepatitis B cirrhosis,which can judge the degree of liver function damage and the prognosis of treatment.The clinical detection significance is significant.
