摘要
目的:观察三维重建联合腹腔镜技术进行精准切除治疗原发性肝癌的临床疗效。方法:随机将80例原发性肝癌患者分为对照组(腹腔镜下进行肝癌的切除)和实验组(对患者肝脏进行三维重建,而后采用腹腔镜下进行肝癌的精准切除)各40例,比较两组患者术中出血量、手术时间、术后住院时间、治疗前后ALT、AST、TBIL、TNF-α、IL-2、IL-6水平。结果:实验组患者术中出血量、手术时间、术后住院时间显著低于对照组患者(P<0.05)。两组患者治疗前后ALT、AST、TBIL、TNF-α、IL-2、IL-6水平差异显著(P<0.05)。治疗后实验组ALT、AST、TBIL、TNF-α、IL-6水平显著低于对照组,IL-2水平显著高于对照组。实验组患者胸腔积液、肺部感染不良反应发生情况显著少于对照组(P<0.05)。结论:三维重建技术联合腹腔镜精准肝切除术治疗原发性肝癌,效果好,有助于保护肝功能,降低炎症状态。
Objective:To investigate the clinical efficacy of three-dimensional reconstruction combined with laparoscopic technique for accurate resection of liver in the treatment of primary hepatocellular carcinoma.Methods:80 patients with primary liver cancer were randomly divided into two groups:control group laparoscopic resection of liver cancer,experimental group.Then laparoscopy was used for precise resection of liver cancer.The amount of intraoperative bleeding,the time of operation,the time of hospitalization after operation,the time of ALT、AST、TBIL、TNF-α、IL-2、IL-6 were compared between the two groups before and after treatment.The statistical difference of IL-6 content in clinical efficacy self-evaluation.Results:the amount of intraoperative bleeding,operative time and postoperative hospital stay in the experimental group were significantly lower than those in the control group(P<0.05).Before and after treatment,there was a significant difference between the two groups(P<0.05)in the content of TNF-α、IL-6、TBIL,and the content of TNF-α、IL-6、TBIL was significantly decreased with the time transfer.TNF-α、IL-6 in the experimental group after treatment The content of IL-2 in the control group was significantly lower than that in the control group.The incidence of side effects of pulmonary infection in the experimental group was significantly less than that in the control group(P<0.05).Conclusion:Three-dimensional reconstruction combined with laparoscopic precise hepatectomy for the treatment of primary hepatocellular carcinoma can help protect liver function and reduce inflammation.
