PDCD4在乳腺癌组织中的表达及与超声征象的相关性

摘要 目的探讨乳腺癌组织中PDCD4的表达及与超声声像图特征间的相关性。方法回顾性分析2016年4月—2019年4月在皖西卫生职业学院附属医院普外科接受诊治的72例乳腺癌患者的临床资料,免疫组化方法检测患者癌组织中PDCD4的表达情况,分析其在不同分期乳腺癌中的表达特点及与相应的乳腺癌超声声像图特征之间的关系。结果乳腺癌组织中PDCD4阳性表达率为43.1%,而癌旁正常乳腺组织表达率为85.0%,二者之间差异有显著性(χ^2=11.04,P=0.001)。随着肿瘤分期的升高,癌组织中PDCD4表达阳性率逐渐下降,Ⅰ+Ⅱ期与Ⅲ+Ⅳ期比较差异有统计学意义(χ^2=4.89,P=0.003)。乳腺癌组织中PDCD4表达下调或缺失与超声所见肿瘤直径>2 cm、微小钙化、后方回声衰减、富血供、淋巴结转移有关(P均<0.05),而与肿块形态、毛刺征无关(P均>0.05)。结论乳腺癌组织中PDCD4表达下调或缺失;PDCD4在乳腺癌组织中的表达与某些超声征象有一定的相关性,超声联合PDCD4检测,有助于为乳腺癌的诊断、治疗及预后评估等提供更多的参考依据。 Objective To explore the expression of PDCD4 in breast cancer tissues and its correlation with ultrasonic features.Methods The clinical data of 72 patients with breast cancer,diagnosed and treated in the general surgery department of the affiliated hospital of western Anhui health vocational college from April 2016 to April 2019,were analyzed retrospectively.The expression of PDCD4 in breast cancer tissues was detected by immunehistochemical method.Its expression characteristics in breast cancer of different stages and its relationship with the corresponding ultrasonic features of breast cancer were analyzed.Results The positive expression rate of PDCD4 in breast cancer tissues was 43.1%and it was 85.0%in adjacent normal breast tissues.There was a significant difference between the two groups(χ2=11.04,P=0.001).With the increase of tumor stage,the positive expression rate of PDCD4 in breast cancer tissues gradually decreased,and there was a significant difference between stage I+II and stage III+IV(χ2=4.89,P=0.003).The decrease or absence of PDCD4 expression in breast cancer tissues was related to the diameter of tumors(over 2 centimeters),micro-calcification,attenuation of posterior echo,blood supply and lymph node metastasis(P<0.05),but not correlated to the shape of tumors and spike sign(P>0.05).Conclusions The expression of PDCD4 is down-regulated or absent in breast cancer tissues.The expression of PDCD4 in breast cancer tissues is correlated with some ultrasonic signs.Ultrasound combined with PDCD4 detection may provide more references for the diagnosis,treatment and prognosis evaluation of breast cancer.
