家庭资源、政策支持与农村贫困老人住院服务利用——基于logit-NB Hurdle模型的微观数据考察

Family Resources,Policy Support and Hospitalization Service Utilization of the Poor Elders in Rural Areas:A Microscopic Data Analysis Based on Logit-NB Hurdle Model

农村贫困老人是疾病经济风险分布密度最高的群体,在健康扶贫政策实施背景下,其医疗服务利用情况值得关注。通过建立负二项分布栅栏模型对农村贫困老人住院服务利用行为进行拟合,分两个阶段重点考察家庭资源和政策支持对是否住院和多次住院次数两个环节的不同影响。研究发现,家庭经济资源是制约农村贫困老人住院服务利用的基本约束,充分的家庭照料资源能够有效减少多次住院行为的发生,短期内健康扶贫政策对农村贫困老人住院服务利用没有产生显著影响。

The rural poor elders are the group with the highest density distribution of disease economic risks,their medical service utilization is worthy of attention.In this paper,the logit-negative binomial hurdle model is used to fit the hospitalization service utilization of the poor elders in rural areas.The family resources and policy support are investigated how to influence whether or not hospitalized and the number of hospitalizations.The study found that economic conditions is the basic constraints that restrict the hospitalization service utilization in the rural poor elders.Full family care can effectively reduce the incidence of multiple hospitalizations.The short-term health poverty alleviation policy has no significant impact on the hospitalization service utilization in the rural poor elders.
