综合护理干预对CO2激光显微手术联合普米克令舒雾化吸入治疗喉接触性肉芽肿的疗效影响

Impact of Comprehensive Nursing Intervention on Effects and Prognosis of Combined Treatment of CO2 Laser Microsurgery and Pulmicort Inhalation

目的观察综合护理干预对CO2激光显微手术联合普米克令舒雾化吸入治疗喉接触性肉芽肿的疗效影响和预后。方法回顾性2011年2月到2017年1月,喉接触性肉芽肿72例患者的临床资料,按照护理方法的不同分为观察组与对照组,每组患者各36例。72例患者均给予CO2激光显微手术联合普米克令舒雾化吸入治疗,治疗4周。治疗期间,观察组患者给予自我护理模式的综合护理干预;对照组,仅给予常规护理。治疗后4周观察两组的疗效,同时比较声带形态学评分。在治疗后1年进行认知功能、躯体功能、社会功能评分、情绪功能评分以及角色功能评分判定。结果所有患者治疗顺利,组织病理学检查均为肉芽组织;观察组与对照组的总有效率分别为100. 0%和86. 1%,观察组优于对照组(P<0. 05)。治疗后声带形态学评分,观察组(1. 24±0. 6)分低于对照组(2. 10±0. 9)分,差异具有统计学意义(P<0. 05);两组评分均低于治疗前(P<0. 05)。观察组治疗后的认知功能、躯体功能、社会功能评分、情绪功能评分以及角色功能评分与对照组相比,明显增高(P<0. 05)。结论综合护理干预应用于CO2激光显微手术联合普米克令舒雾化吸入治疗喉接触性肉芽肿患者,对于患者的声带状况起到明显的改善作用,疗效得到提高的同时改善了患者的生活质量。

Objective To investigate and observe the impact of comprehensive nursing intervention on the effect and prognosis of the combinedtreatment of CO2 laser microsurgery and pulmicort inhalation.Methods Totally 72 patients with laryngeal contact granuloma were equally divided into the observation group and control group bynursing methods. All the patients were treated jointly with CO2 laser microsurgery and pulmicort inhalation,which lasted for 4 weeks.While the patients in the control group being given routine nursing during the treatment period,those in the observation group weregiven the comprehensive nursing intervention in self-care mode on the basis of the nursing care given to the patients in the controlgroup.Results All the patients were treated successfully,and the results of the histopathological examinations showed that all the tissuestreated were granulation tissues. The total effective rates in the observation group and the control group were 100% and 86. 1%respectively,and the rate of the observation group was better than that of the control group(P<0. 05). After the treatment,the vocalcord morphological scores of the observation group and the control group were 1. 24±0. 6 points and 2. 10±0. 9 points respectively,which were lower than those before the treatment(P<0. 05),i. e.,3. 22±0. 4 points and 3. 18±0. 56 points,and the point of theobservation group was lower than that of the control group(P<0. 05). The scores of physical function,role function,social function,emotional function and cognitive function of the observation group were higher than those of the control group after the treatment,andthe differences were statistically significant(P<0. 05).Conclusions Comprehensive nursing intervention in CO2 laser microsurgery combined with pulmicort inhalation therapy of laryngealcontact granuloma can improve the pationt’s vocal cord condition,enhance the treatment effect,promote the improvement of thequality of life of patients.
