前后路联合手术治疗颈椎骨折脱位并脊髓损伤的疗效观察

Clinical Observation of Anterior-posterior Approach Surgery for Cervical Fracture and Dislocation Combined with Spinal Cord Injury

目的分析颈椎骨折脱位并脊髓损伤患者采取前后路联合手术的治疗效果.方法将本院2017年9月~2018年10月收治的72例脊椎骨折脱位并脊髓损伤患者根据治疗方式分为两组,对照组(36例)行单一的后路手术,观察组(36例)开展前后路联合手术.对比两组并发症发生率、神经功能评分(ASIA)及术后脊髓损伤恢复状况(Frankel分级).结果两组D-E级比率和并发症发生率差异有统计学意义(P<0.05);观察组治疗后ASIA评分高于对照组,差异有统计学意义(P<0.05).结论采取前后路联合手术治疗颈椎骨折脱位并脊髓损伤,不仅可以短时间恢复患者脊髓功能,同时有助于避免其他并发症.

Objective To analyze the effect of anterior-posterior approach surgery for cervical fracture and dis?location combined with spinal cord injury.Methods Seventy一two patients with spinal fracture and dislocation combined with spinal cord injury received in our hospital from Sep.2017 to Oct.2018 were divided into two groups according to treatment methods.36 patients in the control group underwent single posterior approach surgery and 36 patients in observation group undenvent anterior-posterior approach surgery.The incidence of complications,ASIA and recov?ery of spinal cord injury(Frankel classification)were compared between the two groups.Results There were signifi?cant differences on D-E grade ratio and incidence of complications between the two groups(P<0.05).The ASIA score in observation group was higher than that in control group after treatment(P<0.05).Conclusion Anterior一posterior approach surgery can not only restore spinal cord function in a short time,but also help to avoid complications.
