摘要
目的探讨血清可溶性程序性死亡因子1(sPD-1)与2型糖尿病(T2DM)患者合并颈动脉粥样硬化(CAS)斑块的相关性。方法选择87例T2DM患者作为研究对象,另外选择33例糖耐量正常者作为对照组。T2DM患者根据有无CAS斑块分为2组:T2DM无CAS组40例和T2DM并CAS组47例。检测并比较各组血清sPD-1浓度和生物化学指标。分析sPD-1与各指标的相关性。用ROC曲线分析sPD-1对T2DM并CAS斑块的预测价值。结果T2DM无CAS组的sPD-1浓度显著高于对照组(P<0.05),T2DM并CAS组的sPD-1浓度显著高于对照组和T2DM无CAS组(P<0.05)。sPD-1与年龄、腰臀比、甘油三酯、空腹血糖、糖化血红蛋白、空腹胰岛素、高敏C反应蛋白呈正相关(P<0.05),与性别(以男性为参照)、高密度脂蛋白胆固醇、脂蛋白(a)呈负相关(P<0.05)。ROC分析提示sPD-1对T2DM并CAS斑块形成的诊断价值为中等(P<0.05)。结论血清sPD-1可能参与T2DM及动脉粥样硬化斑块的形成、发展过程,对T2DM并CAS斑块形成有预测价值。
Aim To investigate the correlation between serum soluble programmed cell death-1(sPD-1)and carotid atherosclerosis(CAS)plaque in patients with type 2 diabetes mellitus(T2 DM).Methods 87 patients with T2 DM were selected as the study subjects,and 33 cases with normal glucose tolerance were selected as the control group.According to the presence or absence of CAS plaque,T2 DM patients were divided into two groups:40 cases in T2 DM without CAS group and 47 cases in T2 DM with CAS group.Serum sP D-1 concentration and biochemical parameters were detected and compared in each group.The correlation between sP D-1 and each index was analyzed.ROC curve was used to analyze the predictive value of sP D-1 for T2 DM with CAS plaque.Results The concentration of sP D-1 in T2 DM without CAS group was significantly higher than that in control group(P<0.05).The concentration of sP D-1 in T2 DM with CAS group was significantly higher than that in control group and T2 DM without CAS group(P<0.05).SPD-1 was positively correlated with age,waist-hip ratio,triglyceride,fasting blood glucose,glycosylated hemoglobin A1,fasting insulin and high-sensitivity C-reactive protein(P<0.05),negatively correlated with gender(male as reference),high density lipoprotein cholesterol and lipoprotein(a)(P<0.05).ROC analysis indicated that the diagnostic value of sPD-1 in T2 DM with CAS plaque formation was moderate(P<0.05).Conclusion Serum sPD-1 may be involved in the formation and development of T2 DM and atherosclerotic plaque,and it has predictive value for T2 DM with CAS plaque formation.
