抗生素不同疗程治疗新生儿肺炎的疗效比较

Comparison of the efficacy of different courses of antibiotics on neonatal pneumonia

目的比较抗生素不同疗程治疗新生儿肺炎的临床疗效。方法168例肺炎患儿,随机分为对照组和观察组,每组84例。观察组患儿接受连续4 d头孢曲松联合阿米卡星治疗,对照组患儿接受连续7 d头孢曲松联合阿米卡星治疗。比较两组患儿治疗前后C反应蛋白(CRP)、降钙素原(PCT)水平及治疗成功率。结果治疗前及治疗后,两组患儿CRP和PCT水平比较差异均无统计学意义(P>0.05);治疗后,观察组CRP(23.8±19.4)mg/L、PCT(1.2±0.9)ng/ml均低于治疗前的(65.4±34.1)mg/L、(8.5±4.5)ng/ml,对照组CRP(20.7±15.8)mg/L、PCT(1.1±0.8)ng/ml均低于治疗前的(63.9±28.9)mg/L、(8.9±5.2)ng/ml,差异均有统计学意义(P<0.05)。两组治疗成功率均为100%,比较差异无统计学意义(P>0.05)。结论接受连续4 d和连续7 d抗生素治疗的新生儿肺炎患儿均能有效降低患儿炎症指标,疗效相似。

Objective To compare the clinical efficacy of different courses of antibiotics on neonatal pneumonia.Methods A total of 168 cases of neonatal pneumonia were randomly divided into control group and observation group,with 84 cases in each group.The children in the observation group received 4 d,while the children in the control group received ceftriaxone and amikacin for 7 d.Comparison was made on C-reactive protein(CRP)and procalcitonin(PCT)levels before and after treatment and treatment success rate between the two groups.Results There was no statistically significant in levels of CRP and PCT between the two groups before and after treatment(P>0.05).After treatment,the CRP(23.8±19.4)mg/L and PCT(1.2±0.9)ng/ml in the observation group was lower than(65.4±34.1)mg/L and(8.5±4.5)ng/ml before treatment.The CRP(20.7±15.8)mg/L and PCT(1.1±0.8)ng/ml in the control group was lower than(63.9±28.9)mg/L and(8.9±5.2)ng/ml before treatment.Their difference was statistically significant(P<0.05).There was no statistically significant difference in treatment success rate between the two groups(P>0.05).Conclusion Antibiotic treatment for 4 d and 7 d shows similar effect,both can effectively reduce the inflammatory index of neonate pneumonia.
