从临床实习教学现状论整合医学教育的必要性 被引量:18

摘要 在我国现有医学教育体制下,本科医学院校应培养掌握基础理论、基本知识和基本技能的毕业生,"基础厚、口径宽、素质高、能力强"是基本目标。临床实习教学是高等医学教育理论与实践结合的重要环节,可促进医学本科生临床综合能力初步形成并迅速增强。专科化是临床医学发展的必然趋势,临床实习教学主体即医学院校附属医院均有较高专科化水平,专业过度细化、专科过度细划和医学知识碎片化趋势明显,导致实习教学系统性和整体性不足。实习教师及学生也存在教学意识不强等问题。整合医学教育是较理想的解决对策,以器官系统为基础的医学教学模式在理论和现实水平能够增强实习教学系统性,提高教学质量,合理的时间安排也能避免医学生精力投入不足等问题,相对现有医学模式有明显的优越性和必要性。 Training in clinical practice is an essential segment which could combine the theory and practice of clinical medicine.Comprehensive ability of undergraduate medical students would establish initially and enhanced rapidly in this key period.In the existing medical education system of our country,undergraduate medical colleges are expected to master the basic theories,knowledge and skills of clinical medicine.The fundamental goal of medical education should be with"thick foundation,wide caliber,high quality and strong ability".Specialization is an inevitable trend in the development of clinical medicine.The affiliated hospitals of medical colleges which undertake the tasks of training in clinical practice have a high degree of specialization with trends of over specialization,over division and fragmented knowledge,resulting in a systematic lack of practical training in reality.Moreover,trainers and trainees also have problems such as lack of training awareness.Theoretically,organ-system based curriculum is an optional solution for troubles in the present situation of clinical practical training for its systemic design.And the advantages and necessity of this model also demonstrated by some practice.
