下颌升支纵形离断入路切除颞下窝肿瘤临床体会

Submandibular Incision with Longitudinal Mandibular Osteotomy Approach for the Treatment of Tumors in Infratemporal Fossa

目的:探讨下颌下切口结合下颌升支纵形离断入路切除颞下窝肿瘤的疗效。方法:回顾性分析2015-01—2018-02期间,我科收治的7例颞下窝肿瘤患者的临床资料。所有患者均采用下颌下切口结合下颌升支纵形离断入路切除肿瘤,随访术后并发症和肿瘤复发情况。结果:术野显露良好,肿瘤完整切除,无下牙槽神经损伤。随访6~36个月,1例迷走神经副神经节瘤患者出现声音嘶哑和饮水呛咳,余6例未出现相关并发症,无肿瘤复发。结论:下颌下切口结合下颌升支纵形离断入路切除颞下窝肿瘤,具有创伤小、创口暴露充分、术后并发症少等优点,值得临床推广应用。

Objective:To analyze the effect of the tumor resection in infratemporal fossa by submandibular incision with longitudinal mandibular osteotomy approach.Methods:Between January 2015 and February 2018,7 patients with tumors in infratemporal fossa were treated by submandibular incision with longitudinal mandibular osteotomy approach.The patients were followed up to evaluate the complications and tumor recurrence.Results:All 7 patients had complete tumor resection with functional preservation of the inferior alveolar nerve.Postoperatively,there were no evidence of tumor recurrence and technique-related morbidities at 6 months to 3 years follow-up.Conclusion:This novel technique is useful for providing excellent access to tumors in infratemporal fossa.
