腹腔镜胆囊切除术联合胆道探查术治疗胆囊结石的临床疗效及对患者QOL评分的影响

Clinical Efficacy of Laparoscopic Cholecystectomy Combined with Bile Duct Exploration for Gallstone Patients and Its Impact on QOL Score

目的:探究腹腔镜胆囊切除术联合胆道探查术治疗胆囊结石患者的临床疗效及对患者QOL评分的影响。方法:选取2014年5月-2019年5月本院收治的胆囊结石患者92例。依据随机数字表法将其分为对照组与研究组,每组46例。对照组采用临床常规性开腹术式治疗,研究组采用腹腔镜胆囊切除术联合胆道探查术治疗。比较两组手术指标、QOL评分、临床疗效及术后并发症发生情况。结果:研究组手术时间、肛门排气时间、首次排便时间、住院时间均短于对照组(P<0.05);术后,研究组生理状态、自觉症状、心理状态、社会功能评分及QOL总分均高于对照组,差异均有统计学意义(P<0.05);研究组总有效率97.8%,高于对照组的78.3%(P<0.05);研究组并发症发生率6.5%,低于对照组的34.8%(P<0.05)。结论:临床治疗胆囊结石患者采用腹腔镜胆囊切除术联合胆道探查术的效果更佳,可提升手术治疗效果,改善预后生活质量,缩减住院时间,降低术后并发症发生率,安全可靠,值得在临床中推广。

Objective:To explore the clinical effect of laparoscopic cholecystectomy combined with exploration of bile duct in the treatment of gallstone and the influence on QOL score.Method:A total of 92 patients with gallstone admitted to our hospital from May 2014 to May 2019 were selected and divided into control group and study group according to the random number table,46 cases in each group.The control group was treated with routine laparotomy,while the study group was treated with laparoscopic cholecystectomy combined with bile duct exploration.The operation index,QOL score,clinical effect and postoperative complications were compared between the two groups.Result:The operation time,anal exhaust time,first defecation time and hospitalization time of the study group were shorter than those of the control group(P<0.05).After operation,the scores of physiological state,conscious symptom,psychological state,social function and total QOL score in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The total effective rate of the study group was 97.8%,which was higher than 78.3%of the control group(P<0.05).The incidence of complications in the study group was 6.5%,which was lower than 34.8%in the control group(P<0.05).Conclusion:Laparoscopic cholecystectomy combined with bile duct exploration is more effective in the treatment of gallstone.It can improve the effect of operation,improve the quality of life,reduce the length of stay,reduce the incidence of postoperative complications.It is safe and reliable,worth popularizing in clinical practice.
