HHFNC联合枸橼酸咖啡因静脉注射治疗早产儿反复呼吸暂停的临床研究 被引量:1

Clinical Study on HHFNC Combined with Intravenous Injection of Caffeine Citrate in Treatment of Premature Infants with Recurrent Apnea

摘要 目的:探讨经鼻导管高流量加温湿化正压通气(HHFNC)联合枸橼酸咖啡因静脉注射治疗早产儿反复呼吸暂停的临床效果。方法:选取2017年8月-2019年6月本院收治的反复呼吸暂停患儿66例。按照随机数字表法将其分为观察组和对照组,各33例。两组均接受枸橼酸咖啡因静脉注射,对照组给予持续气道正压通气(nCPAP)治疗,观察组给予HHFNC治疗,比较两组治疗效果、治疗相关指标及不良反应情况。结果:观察组治疗总有效率93.94%,高于对照组的75.76%(χ^2=4.243,P=0.039);观察组呼吸暂停发生次数少于对照组,消失时间及住院时间均短于对照组,差异均有统计学意义(P<0.05);观察组不良反应总发生率9.09%,低于对照组的36.36%(χ^2=6.988,P=0.008)。结论:HHFNC结合枸橼酸咖啡因静脉注射治疗,可有效提高反复呼吸暂停早产儿的临床效果,减少呼吸暂停发生次数,缩短其消失时间,降低不良反应发生率,促进患儿康复。 Objective:To investigate the clinical effects of humidified high-flow nasal cannula(HHFNC)combined with intravenous injection of Caffeine Citrate in treatment of premature infants with recurrent apnea.Method:A total of 66 premature infants with recurrent apnea admitted to our hospital from August 2017 to June 2019 were selected.According to the random number table method,they were divided into observation group and control group,33 cases in each group.Both groups received intravenous injection of Caffeine Citrate.The control group received nasal continuous positive airway pressure(nCPAP)and the observation group received HHFNC.The therapeutic effects,treatment-related indicators and adverse reactions were compared between the two groups.Result:The total effective rate of the observation group was 93.94%,which was higher than 75.76%of the control group(χ^2=4.243,P=0.039).The incidence of apnea in the observation group was less than that in the control group,and the disappearance time and hospitalization time were shorter than those in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 9.09%,which was lower than 36.36%in the control group(χ^2=6.988,P=0.008).Conclusion:HHFNC combined with caffeine citrate intravenous injection can effectively improve the clinical effect of preterm infants with recurrent apnea,reduce the frequency of apnea,shorten the disappearance time,reduce the incidence of adverse reactions,and promote the rehabilitation of children.
