急性心肌梗死后心力衰竭患者治疗前后BNP水平变化及其与心功能的相关性

摘要 目的:分析急性心肌梗死(AMI)后心力衰竭患者治疗前后B型钠尿肽(BNP)水平变化及其与心功能的相关性。方法:选取90例AMI患者作为研究对象,另选取同期在本院体检的50例健康志愿者作为对照组,所有AMI患者予以常规抗心力衰竭治疗。将AMI患者依据心功能分级再分成三个亚组,比较治疗前后三个亚组和对照组的血浆BNP水平、左室射血分数(LVEF)、左室舒张末内径(LVEDD),采用Pearson分析BNP与LVEF、LVEDD之间的相关性,随访6个月,比较血浆BNP水平≥250 pmol/L与血清BNP水平<250 pmol/L的患者心血管不良事件的发生率。结果:治疗前,心功能II级、III级及IV级患者的BNP水平、LVEDD均明显高于对照组,LVEF明显低于对照组,且BNP水平及LVEDD随着心功能分级的增加而明显升高,LVEF随心功能分级的增加而明显降低;治疗后,心功能II级、III级及IV级患者的BNP水平、LVEDD均较治疗前明显降低,LVEF明显升高,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,血浆BNP水平与LVEF呈显著负相关,与LVEDD呈显著正相关(P<0.05);血浆BNP水平≥250 pmol/L患者的心血管不良事件的发生率为34.92%(22/63),明显高于血浆BNP水平<25 pmol/L患者的11.11%(3/27),差异有统计学意义(P<0.05)。结论:血浆BNP水平与LVEF呈显著负相关,与LVEDD呈显著正相关。
