奥美拉唑联合奥曲肽治疗消化性溃疡合并上消化道出血的临床疗效分析

Clinical Efficacy of Omeprazole Combined with Octreotide in the Treatment of Peptic Ulcer Complicated with Upper Gastrointestinal Hemorrhage

目的研究探讨临床用奥曲肽与奥美拉唑联合对消化性溃疡合并上消化道出血的治疗效果。方法选取2016年2月-2018年10月该院接受的140例消化性溃疡合并上消化道出血患者,按随机数字表法分为对照组、观察组,每组患者70例。对照组患者给予奥曲肽治疗,实验组给予奥曲肽与奥美拉唑联合治疗,对比分析两组患者治疗效果。结果治疗12、24、48 h后,观察组止血成功率分别为81.43%、91.43%、98.57%,均显著高于对照组(χ^2=14.125、13.746、22.943,P<0.05)。观察组治疗总有效率(94.29%)显著高于对照组(75.71%)(χ^2=9.468,P<0.05)。观察组患者的住院时间(7.52±0.54)d、大便隐血时间(5.10±0.53)d、出血停止时间(35.12±8.73)h均少于对照组,差异有统计学意义(t=9.269、11.416、9.242,P<0.05)。结论奥美拉唑联合奥曲肽治疗消化性溃疡合并上消化道出血的效果优于单独使用奥曲肽治疗的效果,可提高溃疡治疗总有效率、止血成功率,缩短住院时间、大便隐血时间、出血停止时间,可以在临床上进行推广应用。

Objective To investigate the therapeutic effect of clinical octreotide combined with omeprazole on peptic ulcer combined with upper gastrointestinal bleeding.Methods A total of 140 patients with peptic ulcer complicated with upper gastrointestinal hemorrhage received from the hospital from February 2016 to October 2018 were enrolled in the control group and observation group according to the random number table.There were 70 patients in each group.Patients in the control group were treated with octreotide,and the experimental group was treated with octreotide and omeprazole.The therapeutic effects of the two groups were compared.Results After treatment for 12,24 and 48 h,the success rate of hemostasis in the observation group was 81.43%,91.43%and 98.57%,respectively,which were significantly higher than the control group(χ^2=14.125,13.746,22.943,P<0.05).The total effective rate of treatment in the observation group(94.29%)was significantly higher than that in the control group(75.71%)(χ^2=9.468,P<0.05).The hospitalization time(7.52±0.54)d,fecal occult blood time(5.10±0.53)d,and bleeding stop time(35.12±8.73)h were lower in the observation group than in the control group,and the difference was statistical significane(t=9.269,11.416,9.242,P<0.05).Conclusion The efficacy of omeprazole combined with octreotide in the treatment of peptic ulcer with upper gastrointestinal hemorrhage is better than that of octreotide alone.It can improve the total effective rate of ulcer treatment,the success rate of hemostasis,shorten the length of hospital stay,occult blood time and bleeding stop time,which can be promoted and applied clinically.
