显微镜下经侧裂岛叶入路治疗高血压性基底节区脑出血的效果 被引量:4

摘要 目的探讨显微镜下经侧裂岛叶入路清除高血压性基底节区脑出血的技术要点及疗效。方法回顾性分析2013年1月—2015年1月11例有明确高血压史且经头颅CT确诊的基底节区脑出血患者的临床资料,病人均采用显微镜下经侧裂岛叶入路清除血肿。结果所有患者术后复查CT血肿清除情况均≥70%。术后随访3~24个月,根据GOS预后评分,恢复良好6例,中残3例,重残1例,植物生存1例,无死亡病例。结论显微镜下经侧裂岛叶入路治疗高血压性基底节区脑出血是一种有效的治疗方式。 Objective To explore the technical essentials and therapeutic effect of microsurgical removal of hypertensive cerebral hemorrhage in the basal ganglia through lateral fissure-insula approach.Methods The clinical data of 11 patients with a clear history of hypertension and basal ganglia cerebral hemorrhage confirmed by head CT from January 2013 to January 2015 were retrospectively analyzed.All patients underwent hematoma evacuation via the lateral fissure-insula approach under the microscope.Results CT reexamination after operation showed the hematoma clearance rate was≥70%in all patients.The patients were followed up for 3 to 24 months after operation.According to the GOS prognosis score,6 cases recovered well,3 cases were moderately disabled,1 case was severely disabled,1 case was plant survival,and there were no deaths.Conclusion It is an effective way to treat hypertensive cerebral hemorrhage in the basal ganglia via the lateral fissure-insula approach under the microscope.
