手足口病流行期疱疹性咽峡炎117例的临床分析

Clinical analysis of 117 cases of herpetic angina in the epidemic of hand, foot and mouth disease

目的分析手足口病流行期疱疹性咽峡炎临床特点及致病原。方法以117例2018年3月至2019年2月在我院接受治疗的疱疹性咽峡炎患儿为研究对象,回顾性分析患儿临床资料,包括临床实验室检测结果、病原检测、临床表现等。结果所纳入117例患儿中,35例患儿CA16及EV71检测结果为阴性,阴性率为29.91%,CA16阳性患儿39例,CA16阳性率33.33%,EV71阳性患儿43例,EV71阳性率为36.75%。CA16阳性组及EV71阳性组患儿CK-MB、BUN、ALT、血沉、hs-CRP、血糖及WBC、持续高热发生率差异不显著(P>0.05),两组患儿呼吸系统表现、循环系统表现、神经系统表现等发生率均存在显著差异(P<0.05)。重症患儿占9.40%,CA16感染2例,占18.18%,EV71感染9例,占81.82%,均表现出神经系统症状,EV71感染率明显高于CA16感染率(P<0.05)。结论疱疹性咽峡炎与手足口病相互平行,主要致病原包括CA16及EV71,加强实验室指标检测并早期识别疱疹性咽峡炎有助于控制病情进展。

Objective To analyze the clinical features and pathogens of herpetic angina in the epidemic period of hand, foot and mouth disease. Methods A total of 117 children with herpes angina treated in our hospital from March 2018 to February 2019 were retrospectively analyzed clinical data, including clinical laboratory test results, pathogen detection, and clinical Performance and so on. Results Of the 117 children, 35 were negative for CA16 and EV71, 29.91 % for negative, 39 for CA16 positive, 33.33 % for CA16, and 43 for EV71, EV71 positive. The rate is 36.75 %. There was no significant difference in the incidence of CK-MB, BUN, ALT, erythrocyte sedimentation rate, hs-CRP, blood glucose and WBC between patients with CA16 positive group and EV71 positive group(P>0.05). Respiratory system performance and circulatory system in the two groups. There were significant differences in the incidence of performance and nervous system performance(P<0.05). Severe cases accounted for 9.40 %, CA16 infection in 2 cases, accounting for 18.18 %, EV71 infection in 9 cases, accounting for 81.82 %, all showed neurological symptoms, EV71 infection rate was significantly higher than CA16 infection rate(P<0.05). Conclusions Herpetic angina is parallel with hand, foot and mouth disease. The main pathogens include CA16 and EV71. Strengthening laboratory indicators and early identification of herpetic angina can help to control the progression of the disease.
