摘要
目的:采用Meta分析方法评估低分子肝素(Low Molecular Weight Heparin,LMWH)对高脂血症胰腺炎(hyperlipidemic pancreatitis,HLP)临床疗效。方法:通过计算机检索PubMed、Embase、Cochrane图书馆等外文数据库和万方数据库、中国知网系列数据库、中国生物医学文献数据库等中文数据库,全面收集2014-2018年低分子肝素对高脂血症胰腺炎临床随机对照试验文献,应用Review Manager 5.3软件进行Meta分析。结果:共纳入10篇中文文献,肝素组共546例,对照组共545例。分析结果显示,肝素组治疗HLP的总有效率95.74%[RR=1.27,95%CI(1.18,1.37),P<0.001]以及显效率68.60%[RR=1.64,95%CI(1.39,1.93),P<0.001]均优于对照组,血清淀粉酶恢复[MD=-1.12,95%CI(-1.43,-0.81),P<0.001]、腹痛缓解[MD=-0.64,95%CI(-0.85,-0.42),P<0.001]和住院时间[MD=-2.32,95%CI(-3.20,-1.44),P<0.001]均低于对照组,以及TG水平[MD=-5.17,95%CI(-7.21,-3.13),P<0.001]低于对照组。结论:低分子肝素合并常规临床治疗对高脂血症胰腺炎有显著的效果。
Objective: To explore the clinical efficacy of Low Molecular Weight Heparin(LMWH) in hyperlipidemic pancreatitis(HLP) by Meta analysis. Method: Through computer retrieval of foreign language databases such as PubMed, Embase, Cochrane library and Chinese databases such as Wanfang database, China knowledge network series database and China biomedical literature database, literatures on clinical randomized controlled trials of LMWH on hyperlipidemia pancreatitis from 2014 to 2018 were comprehensively collected, and Meta analysis was conducted using Review Manager 5.3 software. Result: A total of 10 Chinese literatures were included, 546 cases in the Heparin group and 545 cases in the control group. The analysis results showed that the total effective rate of heparin group was 95.74% [RR=1.27, 95%CI(1.18, 1.37), P<0.001], and the significant efficiency was 68.60% [RR=1.64, 95%CI(1.39, 1.93), P<0.001], serum amylase recovery [MD=-1.12, 95%CI(-1.43,-0.81), P<0.001], and abdominal pain relief [MD=-0.64, 95%CI(-0.85,-0.42), P<0.001] and hospitalization time [MD=-2.32, 95%CI(-3.20,-1.44), P<0.001] were lower than those of the control group, and TG level was lower than that of the control group[MD=-5.17, 95%CI(-7.21,-3.13), P<0.001]. Conclusion: Lmheparin combined with conventional clinical treatment had a significant effect on HLP.
