摘要
目的分析有氧运动联合左卡尼汀对维持性血液透析(maintenance hemodialysis,MHD)患者炎症状态、氧化应激反应和营养状况的影响。方法选取本院2018年6月至2019年3月收治的96例终末期肾病患者为研究对象,将入选患者采用随机数表法分为观察组和对照组,每组各48例。两组患者均接受MHD和左卡尼汀静脉注射治疗,观察组患者在此基础上接受有氧运动干预,两组患者治疗均持续3个月。比较两组患者治疗前后炎性因子、氧化应激、营养状况变化。结果两组患者治疗后血清C反应蛋白(C reactive protein,CRP)、白细胞介素-1(interleukin-1,IL-1)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、丙二醛(malondialdehyde,MDA)、一氧化氮(nitric oxide,NO)水平均显著低于本组治疗前(均P<0.05),谷胱甘肽过氧化酶(glutathione peroxidase,GSH-Px)、白蛋白(albumin,ALB)、前白蛋白(prealbumin,PA)、血红蛋白(hemoglobin,Hb)水平均显著高于本组治疗前(均P<0.05);观察组患者治疗后CRP、IL-1、IL-6、TNF-α、MDA、NO水平均显著低于对照组(均P<0.05),GSH-Px、ALB、PA、Hb水平均显著高于对照组(均P<0.05)。结论有氧运动联合左卡尼汀能够明显改善MHD患者炎症状态、氧化应激反应和营养状况,且安全性值得肯定。
Objective To analyze the effects of aerobic exercise combined with Levocarnitine on inflammatory state,oxidative stress and nutritional status in patients with maintenance hemodialysis(MHD).Method 96 patients with end-stage renal disease admitted to our hospital from June 2018 to March 2019 were randomly divided into observation group and control group,48 cases in each group.All patients were given MHD and levocarnitine intravenous drip.Observation group patients were given aerobic exercise intervention on this basis.The treatment lasted for 3 months in both groups.The changes of inflammatory factors,oxidative stress and nutritional status before and after treatment were compared between the two groups.Result The serum levels of C reactive protein(CRP),interleukin-1(IL-1),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),malondialdehyde(MDA),nitric oxide(NO)were significantly lower than those before treatment(all P<0.05),the levels of glutathione peroxidase(GSH-Px),albumin(ALB),prealbumin(PA),hemoglobin(HB)were significantly higher than those before treatment(all P<0.05).After treatment,the levels of CRP,IL-1,IL-6,TNF-α,MDA,NO in observation group were significantly lower than those in control group(all P<0.05),GSH-Px,ALB,PA and Hb levels were significantly higher than those in control group(all P<0.05).Conclusion Aerobic exercise combined with Levocarnitine can significantly improve the inflammatory state,oxidative stress and nutritional status of MHD patients,and its safety is worthy of affirmation.
