骶髂关节放射学分型及其临床意义

Radiological classification of sacroiliac joint and its clinical significance

目的分析骶髂关节放射学分型和形态特点,为临床诊断提供借鉴。方法回顾性分析2017年1月至2019年1月间在陕西省康复医院放射科行X线骨盆正片者192例的临床及影像资料,所有患者均为普通投照,采用锐柯NOVA型DR成像系统,依据骨盆正位X线片内所观察骶髂关节走行特点和骶髂关节间隙数量行放射学分型。结果依据X线片所观察骶髂关节走行特征和关节特点,男性Ⅰ、Ⅱ、Ⅲ型分布比例分别为8.33%、79.17%、12.50%,女性则分别为26.04%、47.92%、27.08%,差异均有统计学意义(P<0.05);男性在Ⅱ型不同亚型“12”型双间隙、“21”型双间隙、“2”型双间隙及“121”型双间隙间分布比例分别为56.58%、1.32%、1.32%、40.79%,女性则分别为69.89%、2.22%、6.67%、22.22%,差异均有统计学意义(P<0.05);男性在Ⅲ型不同亚型“右侧单间隙,左侧双间隙型”及“右侧双间隙,左侧单间隙型”分布比例分别为75.00%、25.00%,女性则分别为50.00%、50.00%,差异均有统计学意(P<0.05)。结论正常骶髂关节放射学分型中最多见类型为两侧双间隙,而“12”型双间隙所占比例最高,男性、女性在不同类型的构成上有明显差异。

Objective To analyze the radiological classification and morphological characteristics of sacroiliac joint,and to provide reference for clinical diagnosis.Methods The clinical and imaging data of 192 patients undergoing X ray examination in the Department of Radiology,Shaanxi Kangfu Hospital from January 2017 to January 2019 were retrospectively analyzed.All patients were examined by the Ruoke NOVA DR imaging system under ordinary projections.Radiological classification was performed according to the characteristics of sacroiliac joint movement and the number of sacroiliac joint gaps observed in the pelvic X-ray film.Results According to the movement characteristics and joint characteristics observed by X-ray film,the distribution proportion of type Ⅰ,Ⅱ,and Ⅲ was 8.33%,79.17%,and 12.50% in males versus 26.04%,47.92%,and 27.08%in females(P<0.05),respectively.The distribution ratios of"12"double gap,"21"double gap,"2"double gap,and"121"double gap of type Ⅱ were 56.58%,1.32%,1.32%,and 40.79% in males versus 69.89%,2.22%,6.67%,and 22.22%in females,showing statistically significant differences(P<0.05).The distribution ratios of"right single gap and left double gap type"and"right double gap and left single gap type"of typeⅢwere 75.00% and 25.00% in males versus 50.00%and 50.00% in females,showing statistically significant differences(P<0.05).Conclusion The most common type of normal sacroiliac joint is bilateral double gap,and the"12"type double gap has the highest proportion.There are significant differences in the composition of different types of males and females.
