解郁宁神汤治疗郁火脾虚型卒中后抑郁的临床研究

Clinical study on Jieyu Ningshen Decoction in the treatment of post-stroke depression of type of fire stasis and spleen deficiency

目的探讨解郁宁神汤治疗郁火脾虚型卒中后抑郁的临床疗效。方法选取64例郁火脾虚型卒中后抑郁患者并随机分为2组,治疗组给予解郁宁神汤联合黛力新治疗,对照组单纯给予黛力新治疗。观察比较2组治疗前及治疗后2周、4周、8周时汉密尔顿抑郁(HAMD)量表、蒙哥马利和阿斯伯格抑郁症等级(MADRS)量表、美国国立卫生研究院卒中(NIHSS)量表和中医证候积分量表评分的变化情况。结果2组治疗前HAMD评分、MADRS评分、NIHSS评分、中医证候评分比较差异均无统计学意义(P均>0.05),治疗后2组各评分均较治疗前明显下降(P均<0.05),且治疗后4周、8周治疗组HAMD评分、MADRS评分明显低于对照组(P均<0.05),治疗8周后治疗组NIHSS评分明显低于对照组(P<0.05),治疗2周、4周、8周后治疗组中医证候积分明显低于对照组(P均<0.05)。结论解郁宁神汤具有改善卒中后抑郁患者抑郁症状,促进神经功能缺损康复,调理卒中后抑郁中医证候的作用,联合黛力新能够起到协同增效、减毒作用。

Objective It is to investigate the clinical efficacy of post-stroke depression of type of fire stasis and spleen deficiency.Methods Sixty-four patients with post-stroke depression of type of fire stasis and spleen deficiency were selected and randomly divided into two groups.The treatment group was treated with Jieyu Ningshen Decoction combined with Deanxit,and the control group was treated with Deanxit.The changes of HAMD scale,MADRS scale,NIHSS scale,and TCM symptom score scale were observed and compared before treatment,2 weeks,4 weeks,and 8 weeks after treatment.Results There was no significant difference in the scores of HAMD scale,MADRS scale,NIHSS scale,and TCM symptom score scale before treatment between the two groups(P>0.05).All the scores were significantly decreased after treatment in the two groups(P<0.05).The HAMD scores and MADRS scores after 8 weeks of treatment,NIHSS scores after 8 weeks of treatment and the scores of TCM syndrome after 2,4,8 weeks of treatment in the treatment group were significantly lower than those in the control group(P<0.05).Conclusion Jieyu Ningshen Decoction can improve the symptoms of depression,promote the recovery of neurological deficits,and improve TCM syndromes in the treatment of post-stroke depression.Combined with Lixinxin,it can enhance the efficacy and relieve toxicity when combined with Deanxit.
