远程随访健康管理平台在心功能不全患者中的应用研究

Study on Remote Follow-up Health Management Platform in Cardiac Insufficiency Patients

目的探讨使用心血管疾病患者远程随访管理平台对心功能不全患者的影响。方法选择2017年1月—2018年1月来医院就诊的心功能不全患者340例,采用双盲法分为观察组及对照组各172例,观察组采用远程随访管理平台进行管理,应用远程随访管理平台建立患者健康档案,远程实施出院后12个月的随访管理及疗效评估,对照组则采用常规电话随访方式。全部患者于出院后第1周、第1个月、第3个月、第6个月和1年计5个时间点进行一般资料问卷调查。结果两组患者的心功能监测指标比较,N末端B型钠尿肽值较对照组明显下降,左心室射血分数值明显上升,心功能分级有明显改善,差异具有统计学意义(P<0.05)。结论通过运用心血管疾病患者远程随访管理平台对心功能不全的患者进行跟踪随访管理,能够提高患者生活质量。心血管疾病患者远程随访健康管理平台随访管理模式有效,值得应用推广。

Objective To construct remote follow-up management platform for cardiovascular diseases patients,and to explore the effect of the management platform on cardiac insufficiency patients.Methods A total of 340 patients with cardiac insufficiency from January 2017 to January 2018 during hospitalization were divided into the observation group and the control group by double blind method.The observation group was managed by remote follow-up management platform,with the patient health file established by remote follow-up management platform,and the follow-up management and curative effect evaluation carried out remotely 12 months after discharge.The control group was followed up by routine telephone.Results Compared with the control group,the value of N-terminal B-type natriuretic peptide(BNP)was significantly lower,and the left ventricular ejection fraction(LVEF)was significantly improved,and the difference was statistically significant(P<0.05).Conclusion The quality of life of patients with cardiac insufficiency can be improved by using remote follow-up management platform to follow up patients with cardiac insufficiency.The follow-up management model of remote follow-up health management platform for cardiovascular disease patients is effective and worth popularizing.
