摘要
目的探究胃癌和结直肠癌术后患者贫血特点,及贫血与生存时间的相关性。方法收集2011年7月至2015年10月在三峡大学第二人民医院肿瘤科诊断为胃癌、结直肠癌,并行胃癌、结直肠癌手术,符合纳入标准的113例患者,根据原发恶性肿瘤和施术部位分为胃癌组61例和结直肠癌组52例;2组术前行血常规检查以血红蛋白(Hb)90 g/L为界将组内各患者分为正常/轻度贫血组(Hb≥90 g/L)和贫血组(Hb<90 g/L),并对比各组3 a内患者终点事件(死亡)的发生时间,探究各组终点事件与贫血相关性。记录并对比各组术前,术后7 d、1 a、2a、3 a血红蛋白(Hb)的数值。结果术前胃癌组(66.1%)贫血发生率显著高于结直肠癌组(45.8%),P<0.05;贫血对胃癌患者的生存时间无影响(P=0.763);贫血对结直肠癌患者的生存时间有影响(P=0.031);结直肠癌组术前Hb水平显著高于胃癌组(P<0.01),术后7 d 2组无差异,术后1 a、2 a、3 a结直肠癌组Hb水平低于胃癌组(P<0.05)。结论胃癌、结直肠癌患者来说贫血在不同时间段具有不同的特点。对于结直肠癌患者来说,手术后贫血可能更为明显,同时贫血会影响患者的生存率。
Objective To investigate the characteristics of anemia in the gastric and colorectal patients after operation and the correlationship between anemia and the survival time.Me thods The research subjects were 113 patients diagnosed as gastric cancer or colorectal cancer in oncology department of the second people’s hospital of sanxia university,suffering from the corresponding operations and complying with inclusion criteria from July 2011 to October 2015.Ther were divided into gastric cancer group 61 cases and colorectal cancer group 52 cases according to the malignancy in situ and operations.According to hemoglobin level 90 g/L fromregular blood checks,the patients in each group were divided into common/slight anemia group(Hb≥90 g/L)and anemia group(Hb<90 g/L).We compared the death time of died patients between the two group in each group and investigated the correlationship between anemia and the survival time.Recorded and compared Hb levels before operation,7 days,1 year,2 years and3 years after operation.Re s ults The incidence of anemia in the gastric cancer group(66.1%)was higher than that of the colorectal cancer group(45.8%),P<0.05;Anemia had no effect on the survival time of the geastric cancer patients,P=0.763;Anemia had an obvious effect on the survival time of colorectal cancer patients,P=0.031;the Hb level of colorectal group was significantly higher than that of the gastric cancer group before operation,P<0.01;no differences were found 7 days after operation bewteen the two groups;the Hb level of colorectal cancer group were lower than that of gastric cancer group,1 year,2 years,and 3 years after operation,P<0.05.Conclus ion Anemia shows different characteristics in the colorectal cancer group and gastric cancer group at different period.The anemia would be more obvious in the colorectal cancer patients after operation,therefore anemia might affect the survival rate in this group.
