瘢痕子宫再次妊娠合并妊娠期糖尿病对母婴结局的影响

Effect of Scar Uterus Pregnancy with Gestational Diabetes on the Pregnancy Outcomes

目的观察瘢痕子宫再次妊娠合并妊娠期糖尿病对妊娠结局的影响。方法回顾性分析2016年01月至2016年12月海南省妇女儿童医学中心收治的260例瘢痕子宫再次妊娠产妇的临床资料,根据是否合并糖尿病,将研究对象分为观察组和对照组,比较2组母婴结局。结果观察组胎儿宫内窘迫发生率、新生儿窒息发生率、新生儿肺炎发生率高于对照组(P<0.05),新生儿Apgar评分低于对照组,新生儿体重高于对照组(P<0.05),早产儿、胎死宫内发生率无明显差异(P>0.05)。观察组分娩孕周、阴道分娩率低于对照组(P<0.05),产程时间、产时出血量、产褥感染大于对照组(P<0.05)。瘢痕子宫、年龄、BMI、糖尿病家族史是瘢痕子宫再次妊娠合并妊娠期糖尿病的危险因素。结论瘢痕子宫再次妊娠合并妊娠期糖尿病不利于母婴结局,应重点监测,防范风险。

Objective To observe the effect of scar uterus repregnancy with gestational diabetes on the pregnancy outcomes.Me thods The clinical data of 260 cases of re-pregnancy of scar uterus admitted to the Women and Children Medical Center were retrospectively analyzed from January 2016 to December 2016.According to having or no diabetes,the subjects were divided into the observation group and control group,and the maternal and infant outcomes of the two groups were compared.Re s ults The incidences of intrauterine distress,neonatal asphyxia,neonatal pneumonia were higher in the observation group than in the observation group(P<0.05),the neonatal Apgar scores were lower than those in the control group,and the neonatal weights were higher than those in the control group(P<0.05).There was no significant difference in the incidence of premature infants and fetal death(P>0.05).The observation group’s delivered gestational weeks and vaginal delivery rate were lower than those of the observation group(P<0.05);the time of labor,the amount of bleeding during birth,puerperal infection were greater than those in the control group(P<0.05).Scar uterus,age,BMI,and family history of diabetes were risk factors for re-pregnancy and gestational diabetes in scar uterus.Conclus ion Scar pregnancy and pregnancy with gestational diabetes are not conducive to the maternal and child outcomes,which should be monitored for risk prevention.
