上颌前牙区种植相关牙槽骨形态分类 被引量:4

摘要 目的对上颌前牙缺失后的牙槽骨形态进行分类,并分析此分类和不同种植治疗方法之间的相关性,为种植术前评估和治疗方案选择提供依据。方法收集2017年7月至2018年7月就诊于昆明医科大学附属口腔医院口腔种植科的上前牙缺失患者124名,男性66名,女性58名,年龄19~69岁,平均34.85岁。根据缺牙区的影像学特征将牙槽骨形态分为6类,并分析患者接受的不同种植治疗方案与此分类的关系。结果124名患者共181个缺牙位点纳入研究,各类的占比为:第一类Ⅰ亚类17%,第一类Ⅱ亚类40%,第二类Ⅰ亚类15%,第二类Ⅱ亚类28%。其中125个位点71%的位点牙槽骨嵴顶宽度小于5.5 mm。牙槽骨形态分类与缺牙原因无明显相关性(P>0.05)。GBR加种植体同期植入在所有治疗方法中最常用,并在各牙槽骨形态分类中,其运用频率高于其他治疗方法,尤其在第一类Ⅱ亚类和第二类Ⅰ亚类中(P<0.05)。结论上颌前牙区种植相关牙槽骨形态分类可分为6类,且大部分病例嵴顶宽度小于5.5 mm,GBR加种植体同期植入是最常运用的治疗方法. Objective This study classify the alveolar bone morphology after tooth loss on oral cone-beam computed tomography(CBCT)and analyze the correlation between this classification and different implant treatment methods.The purpose is to classify the alveolar bone morphology and provide a basis for pre-implant evaluation and surgical plan selection.Me thods A total of 124 patients with maxillary anterior teeth loss involved in this research[66 males and 58 females aged 19-69 years with a mean age of 34.85 years].According to the imaging features of the CBCT,the alveolar bone morphology were divided into 6 categories.Re s ults A total of 181 edentulous sites in 124 patients were included in the study,Type one sub-typeⅠaccounted for 17%,Type one sub-typeⅡaccounted for 40%,Type two sub-typeⅠaccounted for 15%,Type two sub-type accounted for 28%,and 125 samples(71%)have a bone width of less than 5.5 mm.There is no significant correlation between the classification of alveolar bone morphology and the cause of missing teeth(P>0.05).Implantation with GBR was most commonly used in all treatments and is used more frequently in all classifications,especially in Type one sub-typeⅡand Type two sub-typeⅠ.Conclus ion In patients with missing maxillary anterior teeth,the alveolar bone morphology was divided into 6 categories.Most patients have a bone width of less than 5.5 mm.Implantation with GBR is most commonly used in all treatments.
