锁孔夹闭手术对破裂脑动脉瘤患者术后NF-κB、VEGF水平及脑血管痉挛的影响

Effects of keyhole clipping surgery on the levels of NF-κB, VEGF and cerebral vasospasm in patients with ruptured cerebral aneurysms

目的探讨锁孔夹闭手术对破裂脑动脉瘤患者术后NF-κB、VEGF水平及脑血管痉挛的影响。方法回顾性分析82例破裂脑动脉瘤患者的临床资料,其中42例予以锁孔夹闭术治疗者为A组,40例予以大骨瓣开颅术治疗者为B组。比较两组术后不同时间的NF-κB、VEGF水平和术后7 d的脑血管痉挛发生率。结果两组术后3 d的血清NF-κB、VEGF水平均低于术后1 d,术后7 d的血清NF-κB、VEGF水平均低于术后3 d(P<0.05);A组术后3、7 d的血清NF-κB、VEGF水平均低于B组(P<0.05)。A组术后7 d的脑血管痉挛发生率低于B组(P<0.05)。结论破裂脑动脉瘤患者应用锁孔夹闭术手术可改善NF-κB、VEGF水平,并能降低脑血管痉挛发生率。

Objective To investigate the effects of keyhole clipping surgery on the levels of NF-κB, VEGF and cerebral vasospasm in patients with ruptured cerebral aneurysms. Methods The clinical data of 82 patients with ruptured cerebral aneurysms were retrospectively analyzed. Among them, 42 cases were treated by keyhole clipping surgery as group A, another40 patients were treated by standard large trauma craniotomy surgery as group B. The levels of NF-κB and VEGF at different time after operation and the incidence of cerebral vasospasm at 7 days after operation were compared between the two groups.Results The levels of serum NF-κB and VEGF at 3, 7 days after operation in the two groups were lower than those at the 1 day after operation, and the levels of NF-κB and VEGF at the 7 days after operation were lower than those at the 3 days after operation(P<0.05);the levels of serum NF-κB and VEGF in the group A were lower than those in the group B at 3, 7 days and after operation(P<0.05). At 7 days after operation, the incidence of cerebral vasospasm in the group A was lower than that in the group B(P <0.05). Conclusion The application of keyhole clipping surgery in patients with ruptured cerebral aneurysms can improve the levels of NF-κB and VEGF, and reduce the incidence of cerebral vasospasm.
