曲马多与纳布啡在妇产科日间手术中的麻醉效果

Anesthesia effects of tramadol and naborphine in day surgery of gynecology and obstetrics department

目的探讨曲马多与纳布啡在妇产科日间手术中的麻醉效果。方法选取2018年6月至2019年1月于我院妇产科行日间手术的102例患者作为研究对象,根据麻醉方式的不同将其分为观察组与对照组,各51例。观察组采用纳布啡+丙泊酚静脉麻醉,对照组采用曲马多+丙泊酚静脉麻醉。比较两组患者不同时间点的血流动力学指标、丙泊酚总用量、麻醉苏醒时间、术后不良反应发生情况、VAS评分、Ramsay评分及术后满意度评分。结果两组患者各时间点的MAP、HR、SpO2比较,差异不具有统计学意义(P>0.05)。观察组的丙泊酚总用量少于对照组,麻醉苏醒时间短于对照组(P<0.05);两组患者的呼吸抑制、术中体动及恶心呕吐发生率比较,差异不具有统计学意义(P>0.05)。观察组的VAS评分低于对照组(P<0.05);两组患者的Ramsay评分、术后满意度评分比较,差异不具有统计学意义(P>0.05)。结论曲马多与纳布啡在妇产科日间手术中均可起到安全麻醉的效果,但纳布啡麻醉下患者丙泊酚总用量更少,苏醒时间更短,更适合妇产科日间手术麻醉选择。

Objective To explore the anesthesia effects of tramadol and naborphine in day surgery of gynecology and obstetrics department. Methods From June 2018 to January 2019, 102 patients undergoing day surgery in gynecology and obstetrics department of our hospital were selected as the research objects, and the patinents were divided into observation group and control group according to different anesthesia methods, with 51 cases in each group. The observation group was given naborphine + propofol intravenous anesthesia, and the control group was given tramadol +propofol intravenous anesthesia. The hemodynamic indexes at different time points, total propofol dosage, recovery time of anesthesia, adverse reactions, VAS score, Ramsay score and postoperative satisfaction score were compared between the two groups. Results There were no significant differences in MAP, HR and SpO2 between the two groups(P>0.05). The total dosage of propofol in the observation group was less than that in the control group, and the recovery time of anesthesia was shorter than that in the control group( P < 0. 05);there were no significant differences in the incidence of respiratory inhibition, intraoperative movement, nausea and vomiting between the two groups(P>0.05). The score of VAS in the observation group was lower than that in the control group(P<0.05);there were no significant differences in the scores of Ramsay and postoperative satisfaction between the two groups(P >0.05). Conclusion Tramadol and naborphine can play safe role in the day surgery of gynecology and obstetrics department, but the total dosage of propofol is less and the recovery time is shorter under naborphine anesthesia, which is more suitable for the choice of anesthesia in the day surgery of gynecology and obstetrics department.
