丙种球蛋白治疗儿童传染性单核细胞增多症合并肾小球肾炎的临床疗效

Clinical efficacy of gamma globulin in the treatment of infectious mononucleosis with glomerulonephritis in children

目的:探讨丙种球蛋白治疗儿童传染性单核细胞增多症合并肾小球肾炎的临床疗效。方法:回顾性分析2015年1月至2018年6月期间我院确诊的38例传染性单核细胞增多症合并肾小球肾炎患儿的治疗情况,根据治疗方法的不同分为研究组和对照组,各19例。其中对照组应用更昔洛韦进行治疗,研究组在对照组基础上应用丙种球蛋白进行治疗。比较分析两组患者治疗后临床疗效、外周血T淋巴细胞亚群水平、免疫功能。结果:对照组患者的有效率为57.89%,明显低于研究组患者的有效率94.74%,差异有统计学意义(P<0.05)。对照组患者治疗后的CD3+水平高于研究组,而CD4^+、CD8^+和CD4^+/CD8^+水平低于研究组,差异有统计学意义(P<0.05)。研究组患者治疗后的IgA、IgG、IgM水平明显低于对照组,差异有统计学意义(P<0.05)。结论:应用丙种球蛋白治疗儿童传染性单核细胞增多症合并肾小球肾炎疗效佳,可明显改善患儿的外周血T淋巴细胞亚群水平,提高患儿免疫功能,值得在临床上大力推广。

Objective:To investigate the clinical efficacy of gamma globulin in the treatment of infectious mononucleosis and glomerulonephritis in children.Methods:The treatment of 16 children with infectious mononucleosis and glomerulonephritis confirmed in Huizhou Second Maternal and Child Health from January 2015 to June 2018 was retrospectively analyzed,and were divided into the study group and the control group according to the different treatment methods,19 cases in each group.The control group were treated with ganciclovir,while the study group were treated with gamma globulin on the basis of the treatment used in the control group,with the clinical efficacy,peripheral blood T lymphocyte subgroups and immune function indexes,compared and analyzed.Results:The effective rate of treatment in the study group was 94.74%,which was significantly higher than that in the control group(57.89%),the difference being statistically significant(P<0.05).After treatment,the levels of CD3^+ in the study group were significantly lower than those in the control group,while the levels of CD4^+,CD8^+ and CD4^+/CD8^+ were significantly higher than those in the control group,the difference being statistically significant(P<0.05).After treatment,the levels of IgA,IgG and IgM in the study group were significantly lower than those in the control group,the difference being statistically significant(P<0.05).Conclusion:The application of gamma globulin in the treatment of children's infectious mononucleosis with glomerulonephritis is very effective.It can obviously improve the level of peripheral blood T lymphocyte subgroups and improve the immune function of the children,which is worthy of clinical promotion.
