乳腺癌真空垫联合乳腺托架摆位放疗的疗效及毒副反应分析

Efficacy and Toxic and Side Effects of Vacuum Pad combined with Breast Bracket on Set-up Radiotherapy of Breast Cancer

目的:观察真空垫联合乳腺托架用于乳腺癌摆位放疗对患者摆位误差、疗效、毒副反应等影响。方法:回顾性分析2013年4月至2016年4月于本院接受放疗的86例乳腺癌患者临床资料,按照放疗体位固定方式不同分为观察组45例与对照组41例,观察组采取真空垫联合乳腺托架固定,对照组单用乳腺托架固定,比较两组摆位误差及摆位合格率、心肺照射剂量、放疗毒副反应、近远期疗效。结果:观察组X轴、Y轴摆位误差均显著低于对照组,摆位合格率85.33%显著高于对照组的63.90%(P<0.05);观察组心肺照射剂量均显著低于对照组(P<0.05),放疗相关毒副反应总发生率42.22%,显著低于对照组的78.05%(P<0.05);两组近期疗效、3年内局部复发、远处转移及生存率比较差异均无统计学意义(P>0.05)。结论:真空垫联合乳腺托架用于乳腺癌摆位放疗中,能减小患者摆位误差,降低心肺受量及放疗毒副反应。

Objective:To observe the effects of vacuum pad combined with breast bracketon set-up errors,efficacy and toxic and side effects of patients with set-up radiotherapy of breast cancer.Methods:The clinical data of 86 patients with breast cancer who underwent radiotherapy from April 2013 to April 2016 were retrospectively analyzed.According to the different methods of radiotherapy position fixing,the patients were divided into observation group(45 cases)and control group(41 cases).Observation group was given vacuum pad combined with breast bracket for fixation,and control group was only given breast bracket for fixation.The set-up errors and pass rate of set-ups,cardiopulmonary irradiation dose,toxic and side effects of radiotherapy and short-term and long-term efficacy were compared between the two groups.Results:The set-up errors of X-axis and Y-axis in observation group were significantly lower than those in control group,and the pass rate of set-ups was significantly higher than that in control group(85.33%vs 63.90%)(P<0.05).The cardiopulmonary irradiation dose in observation group was significantly lower than that in control group(P<0.05),and the total incidence rate of radiotherapy-related toxic and side effects was significantly lower than that in control group(42.22%vs 78.05%)(P<0.05).There were no statistically significant differences in the short-term efficacy and local recurrence,distant metastasis and survival rate within 3 years between the two groups(P>0.05).Conclusion:Vacuum pad combined with breast bracket for set-up radiotherapy of breast cancer can reduce the set-up errors reduce the cardiopulmonary irradiation dose and toxic and side effects of radiotherapy.
