微创手术切除与介入栓塞治疗对肝血管瘤患者围术期指标及近期预后的影响

Effects of Minimally Invasive Surgical Resectionand Interventional Embolization on Perioperative Indexes and Short-term Prognosis in Patients with Hepatic Hemangioma

目的:探讨微创手术切除与介入栓塞治疗对肝血管瘤患者围术期指标及近期预后的影响。方法:回顾性分析本院2017年1月至2018年6月60例肝血管瘤患者临床资料,依据手术方式将行微创手术切除患者28例纳入B组,将行介入栓塞治疗患者32例纳入A组,比较两组患者围术期指标与近期预后情况。结果:A组患者围术期指标手术时间与住院时间短于B组,术中出血量少于B组(P<0.05);A组患者总治疗费用高于B组(P<0.05);两组患者治疗后总胆红素(TB)水平比较(P>0.05);A组患者治疗后谷丙转氨酶(ALT)、谷草转氨酶(AST)水平低于B组,白蛋白(ALB)水平高于B组(P<0.05);两组患者术后近期并发症发生率比较(P>0.05)。结论:介入栓塞治疗较微创手术切除治疗肝血管瘤,手术创伤小,患者预后恢复较快,肝功能损伤较小。

Objective:To investigate the effects of minimally invasive surgical resection and interventional embolization on perioperative indexes and short-term prognosis in patients with hepatic hemangioma.Methods:The clinical data of 60 patients with hepatic hemangioma in the hospital from January 2017 to June 2018 were retrospectively analyzed.According to surgical methods,28 patients undergoing minimally invasive surgical resection were included in group B.32 patients undergoing interventional embolization were included in group A.The perioperative indexes and short-term prognosis were compared between the two groups.Results:Theoperation time and hospital stay in group A were shorter than those in group B,and intraoperative blood loss was less than that in group B(P<0.05).The total treatment cost of group A was higher than that of group B(P<0.05).There was no significant difference in level of total bilirubin(TB)between the two groups after treatment(P>0.05).After treatment,levels of alanine aminotransferase(ALT)and aspartate transaminase(AST)in group A were lower than those in group B,while albumin(ALB)level was higher than that in group B(P<0.05).There was no significant difference in incidence of postoperative short-term complications between the two groups(P>0.05).Conclusion:The surgical trauma is smaller,prognosisrecovery is quickly and liver function damage is smaller with interventional embolization in treatment of hepatic hemangioma compared with minimally invasive surgical resection.
