抗凝治疗对下肢骨折低风险组患者深静脉血栓形成的影响

Effect of Anticoagulation Therapy on Deep Venous Thrombosis in Patients with Lower Limb Fracture in Low-Risk Group

目的分析下肢骨折低风险组患者接受抗凝治疗对深静脉血栓形成的影响。方法对2013年2月至2018年11月齐齐哈尔市第一医院收治的80例下肢骨折低风险组患者进行回顾性分析,采用随机分组法将其分为对照组与研究组,各40例。对照组接受深静脉血栓护理预防及物理治疗措施,研究组在对照组基础上接受抗凝治疗。对比两组治疗后的深静脉血栓发生率、血液流变学指标及凝血功能指标。结果经治疗后,研究组深静脉血栓发生率与对照组比较差异无统计学意义(P>0.05);研究组血液流变学指标与对照组比较虽无明显改善(P>0.05),但仍具有一定优势;研究组凝血功能指标低于对照组(P<0.05)。结论抗凝治疗在低风险组下肢骨折患者治疗中的应用,可在一定程度上对患者血液流变学及凝血功能指标进行改善,但对于低风险组患者深静脉血栓的预防效果与传统预防方法无差异,故需在准确判断患者深静脉血栓形成风险性基础上,合理使用抗凝治疗方案。

Objective To study the effect of anticoagulant therapy on deep vein thrombosis in patients with lower extremity fractures in low-risk group.Methods Eighty patients with lower limb fracture in low-risk group admitted to the First Hospital of Qiqihar City from February 2013 to November 2018 were randomly divided into control group and research group,40 cases in each group.The control group received nursing prevention and physical treatment of deep venous thrombosis,while the research group received anticoagulation therapy on the basis of the control group.The incidence of deep venous thrombosis,hemorheological parameters and coagulation function were compared between the two groups after treatment.Results After treatment,there was no significant difference in the incidence of deep venous thrombosis between the study group and the control group(P>0.05);the hemorheological indexes of the study group had no significant improvement compared with the control group(P>0.05),but still had some advantages;the coagulation function indexes of the study group were lower than those of the control group(P<0.05).Conclusion The application of anticoagulant therapy in the treatment of lower limb fracture patients in low-risk group can improve the hemorheology and coagulation function of patients to some extent,but the prevention effect of deep vein thrombosis in patients with low-risk group and traditional prevention methods Differences,it is necessary to rationally use the anticoagulant treatment program based on the accurate judgment of the risk of deep vein thrombosis in patients.
