摘要
目的观察补脾益气药联合泼尼松对自身免疫性炎性肌病大鼠血清激酶的影响。方法通过兔骨骼肌匀浆与完全弗氏佐剂多点注射制成大鼠实验性自身免疫性肌炎模型,并将造模大鼠随机分组为模型组、泼尼松组、补脾益气药组(中药组),泼尼松及补脾益气药联用组(联用组),比较各组与空白组及各组间在药物干预后不同时间血清肌酸激酶(Creatine Kinase,CK)、乳酸脱氢酶(Lactate dehydrogenase,LDH)、(Aspartate aminotransferase,AST)及羟丁酸转移酶(Hydroxybutyric acid dehydrogenase,HBDH)的水平变化。结果Lewis大鼠免疫4周后运动单位时限明显缩短、波幅明显降低(P<0.05),血清CK、LDH、AST、HBDH水平较对照组显著增高(P<0.05);免疫5周后,泼尼松组、联用组CK、LDH、AST、HBDH与同期模型组比较,显著下降(P<0.05);到6周时,泼尼松组、联用组CK、LDH、AST水平均显著低于同周期模型组和中药组(P<0.05),且泼尼松组、联用组6周时AST、HBDH水平显著低于本组5周时AST、HBDH的水平(P<0.05);7周时,中药组CK、LDH、AST、HBDH水平较之前显著下降,较前几周达到最低(P<0.05),联用组LDH、AST水平均显著低于泼尼松组和中药组(P<0.05)。结论泼尼松与补脾益气药均可降低实验性自身免疫肌炎大鼠血清中CK、LDH、AST、HBDH水平,两药合用可能对并发症和预后有所改善。
Objective To investigate the effect of spleen-tonifying and qi-benefiting medicinals combined with Prednisone on serum kinase in rats with autoimmune inflammatory myopathy.Methods Experimental autoimmune myositis rat model was established by multi-point injection of rabbit skeletal muscle homogenate and complete Freund′s adjuvant.The rats were randomly divided into 4 groups:model group,Prednisone group,spleen-tonifying and qi-benefiting medicinals group(TCM group)and combination group.Levels of Serum Creatine kinase(CK),lactate dehydrogenase(LDH),Aspartate aminotransferase(AST),and Hydroxybutyric acid dehydrogenase(HBDH)were compared between all the groups with the blank group and within each of the four groups at different time point after drug intervention.Results After 4 weeks of immunization,the duration of motor unit in Lewis rat model were significantly shortened and the and amplitude significantly decreased(P<0.05),and the levels of serum CK,LDH,AST and HBDH was significantly raised than those in the control group(P<0.05).After immunization for 5 weeks,CK,LDH,AST and HBDH in prednisone group and combination group significantly decreased compared with concurrent model group(P<0.05);at the 6th weeks,CK,LDH and AST levels in prednisone group and combination group were significantly lower than concurrent model group and TCM group(P<0.05),and AST and HBDH levels in prednisone group and combination group were significantly lower than those at the 5th week in the same group(P<0.05);compared with the previous weeks,CK,LDH,AST and HBDH levels in TCM group were significantly decreased and reached the lowest level(P<0.05)after 7 weeks′immunization,besides,the LDH and AST levels in combination group were significantly lower than those in prednisone group and TCM group(P<0.05).Conclusion Both prednisone and spleen-tonifying and qi-benefiting medicinals could reduce the serum levels of CK,LDH,AST and HBDH in experimental autoimmune myositis rats model,and the combination usage of prednisone and spleen-tonifying and qi-benefiting medicinals would improve the prognosis and complications.
