超声引导下小针刀治疗屈指肌腱狭窄性腱鞘炎的临床观察 被引量:13

摘要 目的:观察超声引导下小针刀治疗屈指肌腱狭窄性腱鞘炎(Trigger Finger,TF)的临床疗效。方法:将符合标准的60例患者随机分入治疗组和对照组,观察患者在超声引导行小针刀精准治疗(治疗组)及传统小针刀(对照组)治疗前后、治疗后1周、治疗后1个月的疼痛视觉模拟评分量表(Visual Analogue Scale,VAS)评分、Quinnell分级评定及临床疗效,对数据进行统计学分析,并对其安全性进行评价。结果:前后对照:两组患者治疗前和治疗后、治疗后1周、治疗后1个月VAS疼痛评分、Quinnell分级评定差异均有统计学意义(P <0.05)。组间对照:治疗前对比,两组患者治疗后的VAS疼痛评分、Quinnell分级评定差异无统计学意义(P> 0.05);两组患者治疗后1周、治疗后1个月的VAS疼痛评分、Quinnell分级评定差异有统计学意义(P <0.05)。对照组出现血肿2例,手指麻木1例,安全性低于治疗组。治疗组治愈率76.7%,总有效率100%;对照组治愈率63.3%,总有效率80%,治疗组总有效率高于对照组,两组疗效差异有统计学意义(P <0.05)。结论:超声引导下小针刀治疗屈指肌腱狭窄性腱鞘炎疗效确切,安全性高,值得临床应用推广。 Objective: To observe the clinical effect of ultrasound-guided small needle knife on flexor tendon stenosis tenosynovitis. Methods: 60 patients were randomly assigned to the treatment group and the control group. The treatment group was given ultrasoundguided small needle knife precision treatment. The control group was treated by traditional small needle knife. The VAS score, Quinnell grading, and clinical efficacy were observed and analyzed before and after treatment, one week after treatment, and one month after treatment;the safety was evaluated. Results: There were significant differences in VAS pain scores and Quinnell grading in two groups between before and after treatment, one week after treatment, and one month after treatment(P<0.05). There was no significant difference in VAS pain score and Quinnell classification between groups before treatment(P>0.05). After treatment of one week and one month, there were significant differences in VAS pain scores and Quinnell grading between two groups(P<0.05). In control group, there were two cases of hematoma and one case of finger numbness;the safety was lower than treatment group. In the treatment group, the cure rate was 76.7%;the total efficiency was 100%;those were 63.3% and 80% in the control group. The total efficiency in the treatment group was higher than the control group, with a statistically significant difference(P<0.05). Conclusion: Ultrasound-guided small needle knife is effective and safe on flexor tendon stenosis tenosynovitis, which is worthy of clinical application.
