儿童期美克尔憩室并发症的超声诊断价值

Value of Ultrosound in Diagnosis of Complications of Meckel’s Diverticulum in Children

目的探讨超声对儿童期美克尔憩室(MD)并发症的诊断价值。方法回顾性分析2016年1月—2018年6月哈尔滨市儿童医院普外科手术治疗MD病例92例,包括一般情况、病史、术中所见、病理结果,MD并发症超声检查要点、声像图特征。结果MD并发血便39例,憩室炎24例,肠梗阻15例,憩室穿孔14例,超声检出MD并发血便34例、憩室炎19例、肠梗阻8例、憩室穿孔7例,检出率分别为87.2%、79.2%、53.3%、50.0%。MD超声声像图为右下腹或脐周肠系膜对侧缘指状或囊袋样突起,位置固定,无明显蠕动,一端为盲端,另一端与正常肠腔相通,憩室壁似肠壁回声,黏膜层回声增强、稍厚。结论MD声像图有特征性表现,超声是MD并发出血、憩室炎的有效检查手段。

Objective To evaluate the value of ultrasound in diagnosis of complications of Meckel’s diverticulum(MD)in chil?dren.Methods A total of 92 cases of children with MD who were admitted to Harbin Children Hospital from January,2016 to June,2018 were selected as study subjects.The general clinical data,medical history,pathology results,MD complications ultrasonic testing key points and ultrasonogram were reviewed retrospectively.Results 39 cases had hematochezia,24 cases had diverticulum,15 cases had intestinal obstruction,and 14 cases had perforation.The accuracy of ultrasound of hematochezia,diverticulitis,ob?struction and perforation were 87.2%,79.2%,53.3%and 50.0%.It appeared as cystlike or tubular structure with thick wall,loss of peristalsis,one end blinding and the other end connecting with the gut.Conclusion The sonographic appearances of MD with painless hematochezia or diverticulum were characteristic.Ultrasound is an effective choice to diagnose complications of Meckel’s diverticulum in children.
