中药浸洗防治卡培他滨相关性手足综合征疗效观察

Clinical Observation of TCM Soaking in Preventing and Treating Capecitabine Related HFS

目的:观察中药黄芪桂枝五物汤加味浸洗防治卡培他滨相关性手足综合征的临床疗效。方法:选取病理确诊的晚期肿瘤予卡培他滨单药化疗患者50例,按随机单盲分为治疗组和对照组,每组各25例。对照组采取常规措施预防手足综合征,治疗组采用黄芪桂枝五物汤加味以煎水手足浸洗10 min,2次/d,连用14 d为一个观察周期,共观察2个周期。结果:两组患者相比较,治疗组HFS发生率35%,明显低于对照组68%,差异有统计学意义(P<0.05);治疗组疼痛发生率32%,明显低于对照组64%,差异有统计学意义(P<0.05);治疗组患者NRS评分明显低于对照组,差异有统计学意义(P<0.05);治疗组患者治疗后中医症状学评分改善明显优于对照组,差异有统计学意义(P<0.05)。结论:通过黄芪桂枝五物汤加味浸洗治疗,可使HFS的发生率减低,疼痛症状改善,对化疗药物卡培他滨的耐受性提高,对于确保化疗疗程及疗效、提高患者生活质量起到了积极的作用,值得临床进一步推广及深入研究。

Objective:To observe the clinical efficacy of TCM soaking with modified Huangqi Guizhi Wuwu Decoction in the prevention and treatment of Capecitabine related hand-foot syndrome(HFS).Methods:50 patients with pathologically confirmed advanced tumors received capecitabine monotherapy were randomly divided into the treatment group and the control group,with 25 cases in each group.The control group was treated with routine measures to prevent hand-foot syndrome,whereas the treatment group was also given TCM soaking with modified Huangqi Guizhi Wuwu Decoction(10 minutes each time,twice a day).14 days was taken as one observation cycle,and the treatment duration was two cycles.Results:The incidence of HFS was 35%in the treatment group,which was significantly lower than 68%in the control group(P<0.05).The incidence of pain was 32%in the treatment group,which was also significantly lower than 64%in the control group(P<0.05).The NRS score was significantly lower in the treatment group than that in the control group(P<0.05).The TCM symptom score was improved significantly in the treatment group than that in the control group(P<0.05).Conclusion:TCM soaking with modified Huangqi Guizhi Wuwu Decoction can reduce the incidence of HFS,improve the pain symptom and the tolerance of Capecitabine.It plays a positive role in assuring the course and efficacy of chemotherapy,as well as improving the quality of life of the patients.The therapy is worthy of further clinical promotion and in-depth study.
