摘要
目的对比研究梅毒合并HIV感染者与单纯梅毒、单纯HIV感染者CD8的活化与凋亡水平。方法流式细胞仪检测26例早期梅毒患者,18例HIV感染者,18例早期梅毒合并HIV患者(以下简称合并组),20例健康者外周血单个核细胞(peripheral blood mononuclear cell,PBMC),流式细胞术统计分析四组CD8细胞活化标志物CD38、HLA-DR与凋亡标志物caspase-3水平。结果与健康组相比较,HIV组和合并组CD38阳性率显著升高(5. 97%±0. 66%vs 9. 44%±0. 90%,9. 87%±0. 99%,均P <0. 05);与健康组相比较,HIV组和合并组HLA-DR阳性率显著升高(4. 35%±0. 74%vs 23. 11%±1. 90%,21. 72%±2. 15%,均P <0. 05)。合并组与HIV组比较,caspase-3阳性率升高(13. 09%±2. 37%vs 8. 21%±3. 29%,P <0. 05);合并组与梅毒组(1. 81%±0. 97%)、健康组(0. 24%±0. 17%)相比较,caspase-3阳性率也显著升高(P <0. 001)。HIV组与健康组相比较,caspase-3阳性率显著升高(均P <0. 05)。二期梅毒组caspase-3阳性率明显高于健康组(17. 91%±2. 99%vs 7. 25%±5. 65%,P <0. 05)。结论合并梅毒螺旋体感染可以在一定程度上加剧HIV感染者CD8细胞的凋亡,而这部分凋亡不仅仅是活化诱导的细胞死亡所致。
Objective To compare the activation and apoptosis of CD8 in patients with syphilis and HIV co-infection,early syphilis patients and HIV patients. Methods Flow cytometry was used to detect the activation and apoptosis of CD8 in 26 cases of early syphilis,18 cases of HIV infection,18 cases of early syphilis with HIV,and 20 healthy controls. Results The positive rate of CD8 activation marker CD38 was significantly higher in HIV group and co-infection group than in healthy group( 9. 44% ± 0. 90%,9. 87% ±0. 99% vs 5. 97% ±0. 66%,P < 0. 05). HLA-DR positive rate in HIV group and co-infection group were significantly higher than that in healthy group( 23. 11% ±1. 90%,21. 72% ±2. 15% vs 4. 35% ±0. 74%,P < 0. 05). The positive rate of CD8 apoptosis marker caspase-3 was significantly higher in co-infection group( 13. 09% ± 2. 37%) than in HIV group( 8. 21% ± 3. 29%,P < 0. 05),syphilis group( 1. 81%±0. 97%,P < 0. 001),and healthy group( 0. 24%±0. 17%,P < 0. 001),and it was also significantly higher in HIV group than in healthy group. Caspase-3 positive rate was significantly higher in secondary syphilis group than in healthy group( 17. 91% ±2. 99%vs 7. 25% ±5. 65%,P < 0. 05). Conclusion Infection with Treponema pallidum can aggravate the apoptosis of CD8 cells in HIV-infected patients to a certain extent,and this part of apoptosis is not only caused by activation-induced cell death.
