直肠癌造瘘术后并发改道性结肠炎患者的临床症状及对免疫因子的影响

Clinical Symptoms of Complicated Diversion Colitis after Rectal Cancer Patients Subjected to Ostomy,as well the Influence on Body’s Immune Factors

为探究直肠癌造瘘术后并发改道性结肠炎(DC)患者的临床症状及对免疫因子的影响,选取2014年3月至2019年2月我院收治的73例直肠癌保护性造瘘患者作为观察组,选取同期收治的368例直肠癌未造瘘患者作为对照组,将2组患者临床资料和内镜资料进行比较分析。结果显示,治疗后观察组有21例出现DC相关临床症状,其中腹痛、黏液便、血便症状发生率显著高于对照组,差异具有统计学意义(P<0.05);观察组内镜检出结肠黏膜出现红肿、溃烂、滤泡样增生、炎性息肉、吻合口狭窄发生率均明显高于对照组,差异具有统计学意义(P<0.05);2组外周血T细胞亚群CD3+、CD4+、CD8+、CD4+/CD8+水平比较差异无统计学意义(P>0.05)。结果表明,DC在直肠癌造瘘术后发生率较高,采用内镜检查有助于DC临床早期诊断和治疗。

This study was to investigate the clinical symptoms of complicated diversion coliti(DC)after rectal cancer patients subjected to ostomy,as well the influence on body’s immune factor(IF),enrolled 73 patients who had received protective ostomy in author’s hospital(2014-03-2019-02)during rectal cancer surgery as observation group,at the same time,368 patients with rectal cancer without receiveing ostomy as control group;then compared and analysed both groups’ clinical data and endoscopic findings.As results,the 21 cases of observation group had DC relevant symptoms among which abdominal pain,mucous stool and bloody stool had significantly higher incidence than that of control group(P<0.05);the endoscopic findings of observation group were redness &swelling,ulcerate,follicular hyperplasia,inflammatory polyps,and stoma stenosis which had more significantly higher incidence than that of control group(P<0.05);but in the levels of CD3+,CD4+,CD8+,and CD4+/CD8+ there was no statistical difference between the two groups(P>0.05).Results show that after ostomy DC’s incidence is higher,taking endoscopy can be helpful to early diagnose &treat DC.
