摘要 目的探究白介素-9(IL-9)在椎间盘损伤患者中的表达及其在免疫炎症中的作用机制。方法选择我院2017年3月至2018年12月骨科确诊并收治的椎间盘退行性病变症患者53例作为椎间盘损伤组,另选同期来院诊治的脊柱外伤患者40例作为对照组,检测2组患者椎间盘组织与血清IL-9水平,分析血清IL-9水平与椎间盘损伤程度的关系。将人椎间盘髓核细胞随机分为control组、10 mg/L、50 mg/L与100 mg/L组,除control组外分别给予不同质量浓度IL-9培养24 h,检测各组细胞肿瘤坏死因子-α(TNF-α)蛋白与mRNA表达水平,测定各组细胞培养液中TNF-α与前列腺素E2(PGE2)表达水平。结果椎间盘损伤组患者椎间盘组织IL-9阳性表达率与血清IL-9水平明显高于对照组(P<0.05);Pfirrmann分级为Ⅱ、Ⅲ、Ⅳ级的椎间盘损伤患者血清IL-9水平逐渐升高,各组间差异比较均具有统计学意义(P<0.05);椎间盘损伤患者血清IL-9水平与椎间盘病变程度呈明显正相关(P<0.05);control组、10 mg/L、50 mg/L与100 mg/L组椎间盘髓核细胞TNF-α蛋白与TNF-αmRNA及细胞培养液中TNF-α与PGE2蛋白表达水平逐渐升高,各组间比较差异均具有统计学意义(P<0.05)。结论 IL-9在椎间盘损伤患者中呈高表达;IL-9在椎间盘损伤患者免疫炎症中起促进作用,其机制为上调TNF-α与PGE2表达水平。 To investigate the expression of interleukin-9(IL-9) in patients with intervertebral disc injury and its mechanism of immune inflammation, total of 53 patients with disc herniation(disc injury group) and 40 patients with spinal trauma(healthy control group) were selected in our hospital from March 2017 to December 2018. The levels of IL-9 in intervertebral disc tissues and serum were measured, and the relationship between the serum levels of IL-9 and the degree of intervertebral disc injury was analyzed. Human intervertebral disc nucleus pulposus cells were randomly divided into control group, IL-9(10 mg/L group, 50 mg/L group and 100 mg/L) group. Except the control group, all groups were received corresponding concentration of IL-9 for 24 hours. Then the expression levels of tumor necrosis factor-α(TNF-α) protein and mRNA as well as the prostaglandin E2(PGE2) were detected. Data showed that the positive rate IL-9 expression in intervertebral disc tissue and the level of serum IL-9 in disc injury group were significantly higher than those in healthy control group(P<0.05). The level of serum IL-9 in intervertebral disc injury patients increased with Pfirrmann grade(grade Ⅱ <grade Ⅲ <grade Ⅳ, P<0.05).Furthermore, serum IL-9 level was positively correlated with the degree of intervertebral disc disease in patients with disc injury(P<0.05). The protein and mRNA expression of TNF-α in the nucleus pulposus cells treated with IL-9 were increased(10 mg/L group <50 mg/L group <100 mg/L group, P<0.05). Taken together, the high expression of IL-9 promotes immune inflammation in patients with disc injury, and the mechanism is related to up-regulation of TNF-α and PGE2 expression.
