Vaccination of children with congenital biliary atresia

目的研究先天性胆道闭锁(Congenital biliary atresia,CBA)儿童的预防接种。方法收集整理CBA儿童病例资料,组织临床儿科、流行病学、疫苗学等领域专家召开讨论会,形成CBA儿童预防接种建议,并在北京市顺义区实施,观察接种后不良反应。结果经专家论证,CBA儿童术后肝功能指标虽未达到正常值,停用抗生素3个月后可优先接种灭活疫苗;免疫功能检查正常者可接种减毒活疫苗;激素类药物停药1个月后可接种灭活疫苗,停药3个月后可接种减毒活疫苗。2家接种单位对符合接种建议的3名儿童接种了8种疫苗共15剂次,无不良反应发生。结论CBA儿童预防接种建议可行性好,可保证患儿及时、安全地进行预防接种。

Objective To explore vaccination of children with congenital biliary atresia(CBA).Methods We collected clinical data on children with CBA and developed vaccination guidelines for these children af ter consultation with experts in pediatrics,epidemiology,and vaccinology.We used the guidelines to inf orm vaccination practices of children with CBA in Shunyi district of Beijing and observed adverse reactions af ter vaccination.Results According to the expert guidelines,children with CBA may receive inactivated vaccines af ter stopping antibiotics for 3 months,even though their liver function indices have not reached normal values af ter surgery;children with CBA may receive attenuated live vaccines if their immune function tests are normal;and children with CBA may receive inactivated vaccines and attenuated live vaccines af ter stopping hormonal drugs for 1 month and 3 months,respectively.Three children with CBA who met the guideline’s criteria received 15 doses of 8 different vaccines in 2 vaccination clinics;no adverse reactions occurred.Conclusion Our vaccination guidelines for children with CBA are feasible and can help ensure timely and safe vaccination for children with CBA.
