产前心理辅导联合健康教育对初产妇焦虑抑郁情绪及分娩方式的影响

Effect of prenatal psychological counseling combined with health education on anxiety and depression of primipara and delivery mode

目的探讨产前心理辅导联合健康教育对初产妇焦虑、抑郁情绪及分娩方式的影响.方法将110例初产妇按随机数字表法分为两组,每组55例.两组均予以常规门诊咨询与健康检查,观察组在此基础上予以产前心理辅导联合健康教育,观察20周.于入组时及产前,采用状态焦虑量表评定焦虑状况;于入组时及产后3d,采用汉密顿抑郁量表评定抑郁改善状况;统计分娩方式.结果两组产前状态焦虑量表评分及产后3d汉密顿抑郁量表评分均较入组时显著降低(P<0.01),观察组显著低于对照组(P<0.01).观察组剖宫产率显著低于对照组(P<0.05),助产分娩率及自然分娩率显著高于对照组(P<0.05或0.01).结论产前心理辅导联合健康教育能有效缓解初产妇的焦虑及抑郁情绪,提高自然分娩率.

Objective To explore the effect of prenatal psychological counseling combined with health education on anxiety,depression and mode of delivery in primipara.Methods A total of 110 primiparas were divided into control group and observation group by random number table method,with 55 cases in each group.Both groups were given routine outpatient consultation and health examinations.On the basis of this,the observation group was given prenatal psychological counseling combined with health education for 20 weeks.At the time of enrollment and prenatal,S-AI was used to assess anxiety.At the time of enrollment and 3 days postpartum,HAMD was used to assess depression improvement;analyze the delivery mode.Results The prenatal S-AI score and the HAMD score of postpartum 3 days were significantly lower than those in the group(P<0.01).The observation group was significantly lower than the control group(P<0.01).The cesarean section rate of the observation group was significantly lower than that of the control group(P<0.05),and the midwifery delivery rate and natural delivery rate were significantly higher than the control group(P<0.05 or 0.01).Conclusions Prenatal psychological counseling combined with health education for primiparas can effectively alleviate anxiety and depression and increase their natural delivery rate.
