定量脑电图对MCA重度狭窄患者脑缺血所致神经功能缺损评估的临床价值 被引量:1

摘要 目的探讨定量脑电图对单侧大脑中动脉重度狭窄患者脑缺血所致神经功能缺损评估的价值.方法选取65例单侧大脑中动脉重度狭窄患者设为观察组,65名健康志愿者设为对照组,均进行定量脑电图检查,对观察组患者采用美国国立卫生研究院卒中量表评定神经功能缺损状况.结果观察组全脑区δ波、θ波相对波段功率显著高于对照组(P<0.05),α波相对波段功率显著低于对照组(P<0.01);两组全脑区β波相对波段功率比较差异无统计学意义(P>0.05).观察组患侧δ波相对波段功率显著高于健侧(P<0.05),α波相对波段功率显著低于健侧(P<0.01),患侧与健侧θ波、β波相对波段功率比较差异无统计学意义(P>0.05).相关分析显示,观察组美国国立卫生研究院卒中量表评分与δ波相对波段功率呈显著正相关(rs=0.722,P<0.05),与α波相对波段功率呈显著负相关(r=-0.611,P<0.05).结论定量脑电图在大脑中动脉重度狭窄评估中有一定应用价值,δ波、α波相对波段功率与患者神经功能缺损程度有一定相关性. Objective To evaluate the value of quantitative electroencephalogram(qEEG)in patients with severe stenosis of unilateral middle cerebral artery(MCA).Methods 65 cases of unilateral MCA severe stenosis were selected as the observation group,and 65 healthy volunteers were selected as the control group,all of them were examined by qEEG.NIHSS was used to assess neurological deficits in the observation group.Results The relative band power(RBP)of δ wave and θ wave in the whole brain area of the observation group were significantly higher than that in the control group(P<0.05),while RBP of α wave was significantly lower than that of the control group(P<0.01).There was no significant difference in RBP of β wave between the observation group and the control group(P〉0.05).The RBP of δ wave in the observation group affected side was significantly higher than that of the unaffected side(P<0.05),while RBP ofαwave was significantly lower than that of the unaffected side(P<0.01).There was no significant difference between the observation group and the control group in the RBP of θ wave,β wave between the affected side and the healthy side(P>0.05).There was a positive correlation between δ wave RBP and NIHSS score(rs=0.722,P<0.05).There was a negative correlation between α wave RBP and NIHSS score(r=-0.611,P<0.05).Conclusions Quantitative EEG has certain application value in the evaluation of severe stenosis of middle cerebral artery.The relative wave power of δ wave and α wave is related to the degree of neurological deficit of patients.
