柔肝化纤颗粒联合骨髓间充质干细胞移植治疗乙肝肝硬化的临床观察 被引量:11

摘要 目的:观察柔肝化纤颗粒联合骨髓间充质干细胞(MSCs)移植治疗乙肝肝硬化的疗效。方法:将90例乙肝肝硬化患者按随机数字表法分为A组(西医内科综合治疗组)、B组(MSCs移植组)、C组(MSCs移植联合柔肝化纤颗粒组)。检测患者治疗前后肝功能、凝血酶原(PT)、HBV DNA载量、表观扩散系数(ADC值)及Child-Pugh。结果:治疗后3组患者谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBIL)、PT水平均低于治疗前,白蛋白(ALB)高于治疗前,且C组患者治疗后AST、ALT、TBIL、PT均低于A组、B组(P<0.05);治疗后3组HBV DNA载量显著下降,且C组治疗后HBV DNA载量低于A组、B组(P<0.05);治疗后3组Child-Pugh评分低于治疗前,且C组治疗后Child-Pugh评分低于A组、B组(P<0.05);3组患者治疗后ADC均高于治疗前(P<0.05),且C组患者治疗后ADC高于A组、B组(P<0.05)。C组总有效率高于A组、B组,B组总有效率高于A组,差异均有统计学意义(P<0.05)。结论:柔肝化纤颗粒联合骨髓间充质干细胞移植治疗乙肝肝硬化能明显改善肝功能、肝硬化程度、凝血功能,并提高临床疗效。 Objective: To observe the therapeutic effect of Rougan Huaxian granule combined with bone marrow mesenchymal stem cells(MSCs) in the treatment of hepatitis B cirrhosis. Methods: A total of 90 patients with hepatitis B cirrhosis were randomly divided into group A(integrated treatment group of western medicine),group B(MSCs transplantation group) and group C(MSCs transplantation combined with liver-softening chemical fiber granules group). Liver function, prothrombin(PT), HBV DNA load, apparent diffusion coefficient(ADC) and Child-Pugh were measured before and after treatment. Results: After treatment, the levels of aspartate aminotransferase(AST), alanine aminotransferase(ALT), total bilirubin(TBiL) and PT were lower than those before treatment, albumin(ALB) was higher than before treatment, and patients in group C were treated. AST, ALT,TBIL and PT were lower than group A and group B(P <0.05). After treatment, the Child-Pugh scores of the three groups were lower than those before treatment, and the Child-Pugh scores of group C were lower than those of group A and group B(P<0.05).The ADC of the three groups after treatment was higher than that before treatment(P<0.05), and the ADC of group C after treatment was higher than that of group A and group B(P<0.05). The total effective rate of group C was higher than that of group A and group B, and that of group B was higher than that of group A, with statistical significance(P <0.05). Conclusion: Rougan Huaxian Granule combined with bone marrow mesenchymal stem cell transplantation in the treatment of hepatitis B cirrhosis can significantly improve liver function, liver cirrhosis degree, coagulation function, and improve clinical efficacy.
