摘要
目的分析纤维蛋白原(FIB)水平在卒中相关感染(SAI)中的诊断作用。方法选取医院神经内科2017年1-6月收治的急性脑梗死住院患者176例为研究对象,记录感染发生情况及血清FIB、超敏C反应蛋白(hsCRP)水平。绘制受试者工作特征曲线(ROC curve),确定最佳界值及诊断敏感度及特异度;采用Pearson相关分析法对FIB及hs-CRP进行分析。结果 FIB与炎性指标hs-CRP呈显著正相关(r=0. 646,P <0. 01);在诊断卒中相关感染中具有良好的效度,AUC为0. 833[95%CI:0. 766-0. 899],P <0. 01,最佳界值:4. 115 g/L,敏感度:73. 5%,特异度:84. 9%。结论 FIB可作为卒中相关感染的诊断指标。
Objective To analyze the value of fibrinogen( FIB) levels in the diagnosis of stroke-associated infection( SAI). Methods One hundred and seventy-six patients with acute cerebral infarction were enrolled,and the incidence of infection and serum FIB and high-sensitivity C-reactive protein( hs-CRP) levels were recorded. Draw a receiver operating characteristic curve( ROC curve) to determine the optimal cut-off value and diagnostic sensitivity and specificity. FIB and hs-CRP were analyzed using Pearson correlation analysis. Results There was a significant positive correlation between FIB and inflammatory index hs-CRP( r = 0. 646,P < 0. 01). It has good validity in the diagnosis of stroke-related infections,AUC:0. 833 [95% CI: 0. 766-0. 899],P < 0. 01,best cut-off value: 4. 115 g/L,sensitivity: 73. 5%,specificity: 84. 9%.Conclusion FIB can be used as a diagnostic indicator for stroke-related infections.
