心悦胶囊对介入后急性心肌梗死患者心功能的影响

Effect of Xinyue Capsule on cardiac function in patients with acute myocardial infarction after percutaneous coronary intervention

目的观察心悦胶囊对介入后急性心肌梗死(AMI)患者心功能的影响及可能机制。方法选取2017年1月~2018年6月,成功实施急诊经皮冠状动脉介入治疗(PCI)的AMI患者130例,按照随机数字表法分为治疗组66例和对照组64例,对照组给予常规西医药物治疗,治疗组在常规西医药物治疗基础上加用中药心悦胶囊,连服6个月。观察所有入组患者心功能分级、血清N末端利钠肽前体(NT-proBNP)水平及超声心动图心功能参数,包括左室舒张末容积(LVEDV)、左室收缩末容积(LVESV)、左室射血分数(LVEF),同时检测血清核因子-κBp65(NF-κBp65)、肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)、丙二醛(MDA)、超氧化物歧化酶(SOD)的水平。结果两组患者在治疗6个月后,NT-proBNP,LVEDV,LVESV,NF-κBp65,TNF-α及MDA均低于治疗前,差异有统计学意义(P<0.05),LVEF,IL-10,SOD高于治疗前,差异有统计学意义(P<0.05);服药6个月后,治疗组患者的NT-proBNP,LVEDV,LVESV,NF-κBp65,TNF-α及MDA低于对照组,差异有统计学意义(P<0.05),LVEF,IL-10,SOD高于对照组,差异有统计学意义(P<0.05)。结论心悦胶囊可保护急诊PCI术后AMI患者心功能,其机制可能与抑制炎症反应及氧化应激反应有关。

Objective To investigate the effect of Xinyue Capsules on cardiac function in patients with acute myocardial infarction(AMI) after percutaneous coronary intervention(PCI) and its possible mechanism.Methods One hundred and thirty patients with AMI after successful PCI from January 2017 to June 2018 in the hospital were divided into treatment group(66 cases) and control group(64 cases) according to random number table method.The control group received routine treatment with western medicine.The control group was treated with regular western medicine,and the treatment group was added with traditional Chinese medicine Xinyue capsule on the basis of regular medicine treatment.Both groups were treated and followed for 6 months.The cardiac function,serum N-terminal pro-Btype natriuretic peptide(NT-proBNP) level and echocardi cardiac function parameters,include the left ventricular end diastolic volume(LVEDV),the left ventricular end systolic volume(LVESV) and the left ventricular ejection fraction(LVEF),serum nuclear facter-Kappa B p65(NF-κBp65),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),malondialdehyde(MDA) and superoxide dismutase(SOD) levels were observed in all patients.Results After the treatment for 6 months,NT-proBNP,LVEDV,LVESV,NF-κBp65,TNF-α and MDA were lower than before in both two groups,and the differences were statistically significant(P<0.05),LVEF,IL-10 and SOD were statistically higher than before(P<0.05).After 6 months of treatment,compared with the control group,NT-proBNP,LVEDV,LVESV,NF-κBp65,TNF-α and MDA were statistically lower in the treatment group(P<0.05),while LVEF,IL-10 and SOD were higher than the control group,and the difference was statistically significant(P<0.05).Conclusion Xinyue Capsule can protect heart function in patients with AMI after emergency PCI,and its mechanism may be related to inhibition of inflammatory response and oxidative stress.
