应用CT测量骨骼肌时观察者内和观察者间的变异性研究

摘要 目的探讨应用CT测量骨骼肌面积时观察者内和观察者间的变异性。方法收集2018年11月至2019年6月该院29例食管/胃癌患者CT影像数据,选取2名放射科医生分别作为观察者A和观察者B,对患者进行骨骼肌面积测量,分析观察者内和观察者间测量变异性。结果29例患者肌肉面积为98~261cm^2,平均156cm^2。所有重复测量的平均绝对差值为0.80cm^2,单层面测量的变异性为2.92cm^2。与单层面相比,双层面重复测量的变异性和绝对差异略有改善。单层面测量中,观察者A的变异性最低,同一层面上成对测量值的百分比最高。观察者间测量变异性约为3cm^2。结论该方法能够重复测量骨骼肌面积,并能够一定程度上减少测量变异。 Objective To explore the intra-and interobserver variability in the measurement of skeletal muscle area with CT.Methods CT image data of 29 patients with esophageal/gastric cancer in the hospital from November 2018 to June 2019 were collected.Two radiologists were selected as observers A and B respectively.Skeletal muscle areas of the patients were measured,and the intra-and interobserver variability of measurements were analyzed.Results The mean muscle area of 29 patients was 156 cm^2,and the range was 98 to 261 cm^2.The mean absolute difference of all repeated measurements was 0.80 cm^2,and the variability of single-layer measurement was 2.92 cm^2.Compared with single-layer surface,the variability and absolute difference of double-layer surface repeated measurement were slightly improved.In monolayer measurement,observer A had the lowest variability and the highest percentage of paired measurements on the same level.The interobserver variability of measurements was about 3 cm^2.Conclusion This method can measure skeletal muscle area repeatedly and reduce measurement variation to a certain extent.
