肝移植治疗成人巨大多囊肝1例报告

Liver transplantation for giant polycystic liver disease:A case report

目的报道1例巨大多囊肝患者行肝移植手术的诊治经过。方法患者女性,38岁,因"发现多囊肝5年,顽固性腹水5个月余"于2017年9月4日入本院肝胆外科,并于2017年9月14日行同种异体肝移植术。结果患者手术顺利,术中腹水18000 ml,切除病肝大小57 cm×18 cm×9 cm,重10.6 kg,给术中肝门区显露和手术操作带来困难。患者术后恢复顺利,第22天出院。随访19个月,肝功能稳定。结论肝移植术是晚期多囊肝患者的最佳治疗手段,手术复杂程度高,患者预后良好。

Objective To report a case with giant polycystic liver disease treated by liver transplantation.Methods A 38-year-old woman admitted to our department on September 4,2017 for"5-year history of polycystic liver disease and refractory ascites for 5 months".The patient underwent liver transplantation on September 14,2017.Results The operation was successful.The size of resected liver lesion was 57 cm×18 cm×9 cm,and the weight was 10.6 kg,which made it difficult to expose the hepatic hilar region.Totally,18000 ml ascites was drained.The patient recovered well and was discharged on the 22nd day after operation.She was followed up for 19 months with stable liver function.Conclusion Liver transplantation is the best treatment for patients with polycystic liver disease,with good prognosis.
