分散式日间手术管理模式下护士工作体验的质性研究

Qualitative research of working experience in the management mode of distributed daytime surgery nurse

目的简述分散式日间手术管理模式下护士的工作体验,探讨改进策略,为更好地建设日间手术护理团队提供参考。方法采用现象学质性研究方法,对15名护士进行半结构式访谈,并运用Colaizzi的7步分析法对访谈资料进行分析。结果经分析得出主题:分散式日间手术管理模式的开展具有一定必要性,存在病种多样、医嘱繁杂、流动性强、护患情绪焦躁的特点,护士面临着专业知识更新不足、健康教育欠落实、医护和医患沟通存在误差、护士绩效期望与实际存在差距、床位空置加重护士心理负担的问题。结论优化日间手术病房患者收治流程及管理体系,确保医患、医护流畅沟通,关注护患心理健康,完善护士薪酬管理体系,健全护士培训及宣教系统可有效帮助分散式日间手术病房护士更好地开展工作。

Objective To briefly describe the work experience of nurses under the decentralized day Operation management mode,and explore Improvement Strategies,so as to provide reference for the better construction of day surgical nursing team.Methods Using phenomenological qualitative research method,Semi-structured interviews were conducted with 15 nurses,and the interview data were analyzed by using Colaizzi′s 7-Step analysis method.Results After analysis,the theme:the development of decentralized day surgery management mode has certain necessity,there are various kinds of diseases,complicated medical advice,strong fluidity,the characteristics of emotional agitation,nurses are faced with the lack of professional knowledge,poor health education,medical care and doctor-patient communication errors,gap between nurses′performance expectation and actual existence,and bed vacancy increases the psychological burden of nurses.Conclusions Optimize the process and management system for patients in day surgical wards,ensure smooth communication between doctors,patients and nurses,pay attention to the mental health of nurses and patients,improve the nurse pay management system,and improve nurse training and education system.These can effectively help decentralized day surgical ward nurses to carry out their work better.
