IgM抗-E影响母亲血型反定型和IgG抗-E致新生儿溶血病1例

Effect of IgM anti-E on reverse typing of maternal blood group and hemolytic disease of newborn caused by IgG anti-RhE: a case report

目的对ABO血型正反定型不合的母亲和其出生7 d严重黄疸的新生儿进行血型血清学试验并分析结果,为临床上开展孕妇产前抗体筛查和新生儿溶血的预防和治疗提供依据。方法对患儿和母亲进行血型鉴定、血清抗体筛查、特异性抗体鉴定及效价测定,并对患儿进行溶血3项试验检测。结果患儿血型为A型、ccDEe,母亲为A型、CcDee。患儿血清和红细胞放散液中均检出IgG抗-E,母血清中检出高效价的IgM和IgG混合型抗-E。结论患儿确诊为母婴RhE不合引起的新生儿溶血病,母亲做产前抗体筛查有助于预防新生儿溶血或早期治疗,减轻患儿受累。

Objective Blood type serological tests were performed on a mother with incompatible ABO typing results and her newborn with severe jaundice 7 days after birth,inorder to provide references for the prevention and treatment of prenatal antibody screening and neonatal hemolysis.Methods Blood type identification,serum antibody screening,specific antibody identification and titer determination were performed on mother and her newborn.Three trials of hemolysis were tested for the newborn.Results The blood types of the mother and newborn were A CcDee and A ccDEe,respectively.IgG anti-E antibody was detected in both neonatal serum and erythrocyte released fluid.High titer IgM and IgG mixed with anti-E antibody were detected in maternal serum.Conclusion The newborn was diagnosed with neonatal hemolytic disease caused by maternal and infant RhE incompatibility.Prenatal antibody screening can help the prevention or early treatment of neonatal hemolytic disease and reduce the involvement of the newborn.
