脑钠肽对慢性充血性心力衰竭患者的预后及危险分层的临床诊断价值

The clinical value of BNP in the prognosis and risk stratification of patients with chronic congestive heart failure

目的:探讨脑钠肽(BNP)对慢性充血性心力衰竭(CHF)患者的预后及危险分层的临床诊断价值。方法:2015年6月-2017年6月收治CHF患者120例,检测血浆BNP水平、左室收缩末期容积(SEV)、左室舒张末期容积(EDV)和左室射血分数(LVEF),进行为期3个月随访。观察并记录患者发生终末事件(再住院率、死亡率)情况,分析BNP对CHF患者预后及危险分层诊断的作用。结果:患者BNP水平随心功能分级增加呈上升趋势,差异有统计学意义(P<0.05)。与SEV、EDV及LVEF水平异常患者相比,各项指标正常患者BNP水平明显较各项指标异常患者更高,差异有统计学意义(P<0.05)。BNP<100 pg/mL患者终末事件发生率明显更低,差异有统计学意义(P<0.05)。结论:BNP水平能反应CHF患者心功能严重程度,在早期临床诊断和预后有显著指导作用。

Objective:To explore the clinical value of BNP(brain natriuretic peptide)in the prognosis and risk stratification of patients with chronic congestive heart failure.Methods:120 patients with chronic congestive heart failure were selected from June 2015 to June 2017,the Plasma BNP levels were measured and left ventricular end-systolic volume(SEV),left ventricular end-diastolic volume(EDV),and left ventricular ejection fraction(LVEF)were measured,all patients were followed up for 3 months.We observed and recorded the terminal event(re-hospitalization rate,mortality)in patients,and analyzed the effect of BNP on the prognosis and risk stratification of CHF patients.Results:The BNP level of patients increased with the increase of heart function grade,the difference was statistically significant(P<0.05).Compared with patients with abnormal levels of sev,EDV and LVEF,the BNP level of patients with normal indexes was significantly higher than that of patients with abnormal indexes,the difference was statistically significant(P<0.05).The terminal event rate of patients with BNP<100 pg/mL was significantly lower,and the difference was statistically significant(P<0.05).Conclusion:BNP level can reflect the severity of heart function in patients with chronic congestive heart failure,and has a significant guiding role in early clinical diagnosis and prognosis.
